Case Manager on Clayton Mobile Support and Treatment Team

About the role
Our Monash Health Mobile Support Team Case Managers work within a multi-disciplinary team to provide treatment and support to individuals experiencing complex mental health problems. Our Case Managers work with a limited number of cases to provide intensive support to clients in the community. We strive to deliver high quality recovery-focussed client care. Our aim is to promote self-efficacy and independence, and social inclusion with all the individuals we support.
You will enjoy the challenges of working with the complexities of mental health and making a positive difference every day. You will work closely with the range of mental health services to effectively co-ordinate client care, including inpatient units, CATT, CCU and PARCS.
What you need to know
The position involves rotating daytime roster with occasional rostered weekend shifts.
Community Mental Health Service located in Clayton, close to CBD and South East Sector.
Clients seen in the clinic setting as well as in their own home environment
Autonomous role with the balance of working in a team environment
Access to high quality clinical supervision offered
You will be a Registered Psychiatric Nurse, Registered Occupational Therapist, or Social Worker, with experience in providing community mental health services (Nurses must have completed either a post grad diploma in Psychiatric / Mental Health Nursing or in a specialist undergraduate psychiatric nursing program).
You understand the complexities of working in Mental Health through your demonstrated and existing experience. You are an innovative thinker and passionate about taking part in achieving excellence. You are committed to high quality, patient centred and recovery-focussed care. You are a strong team player, hard working and enthusiastic and willing to contribute to the continual improvement of the service.
