About The Role:
Monash Health Clayton Continuing Care Team case managers work within a multi-disciplinary team to provide treatment and support to adult clients experiencing mental health issues. Our case managers work with an allocated caseload to address the biopsychosocial needs of clients on a person centred, recovery based model. Our aim is to delivery high quality client care that promote self-efficacy, independence and community integration.
You will enjoy the challenges of working with the complexities of mental health to improve individuals quality of life and to make a positive difference every day. You will work closely with a range of mental health services to effectively co-ordinate client care, including inpatient units, CATT, CCU, PARCS, Mental Health Support and Drug and Alcohol services.
What You Need to Know:
The position involves Monday - Friday daytime roster.
Clayton Community Mental Health Service is located close to the CBD and the Sout East sector.
Clients are seen in the clinic as well as in their own home environment.
It is an autonomous role with the balance of working in a team environment
Access to high quality clinical supervision is offered
You will be a Registered Psychiatric Nurse, Registered Occupational Therapist or Social Worker with experience in providing community mental health services (Nurses must have completed either a post grad diploma in Psychiatric/Mental Health Nursing or in a specialist undergraduate psychiatric nursing program)
You understand the complexities of working in Mental Health through your demonstrated existing experience. You are an innovative thinker and passionate about taking part in achieving excellence. You are committed to high quality, client centred and recovery focused care. You are a strong team player, dedicated and willing to contribute to the continual improvement of the service.
