About the role:
The Fono has an exciting opportunity for a registered nurse who is passionate about making a difference in Pacific Health. The ‘RN Outreach’ role works alongside the general practice team and dedicated social workers to meet the health needs of patients and families who encounter barriers to accessing general practice services.  The role provides essential medical services to our most vulnerable patients in their homes, supporting and helping transform them towards their healthiest lives.

An RN’s ability to achieve more is enhanced by The Fono’s integrated model of care “FonoCare”.  The Primary Health outreach RNs is part of a general practice team with not only GPs, HCAs, and Social Workers, but also mental health clinical specialists, Health Coaches, Public Health services, Dental services, Whanau Ora, and a wide range of social services lifting the economic and wellbeing of Pacific families.

  • Primary health and/or community care experience
  • Must hold current registration with Nursing Council of New Zealand.
