About the role

An exciting opportunity is available within theGEM@Hometeam which is expanding to includeRehab@Home. Primarily based at Wantirna Health GEM@Homeprovides flexible high quality services to patients that have conditions associated with ageing, cognitive dysfunction and Chronic illness. The team is now expanding to include adults aged 18 or over requiring short term goal directed care with a focus on Rehab. This is a great opportunity for an enthusiastic and motivated Grade 3 Physiotherapist to be a leader within a multi-disciplinary team. The successful candidate will be able to apply senior decision making to support and develop the team of Grade 2 Physiotherapists, AHA's and under-graduate students as they provide evidence-based care to theGEM@Home/Rehab@Homepatients in their own community setting.

You will be a dynamic, enthusiastic team player within the multi-disciplinary team and Eastern Health Physiotherapy. You will have excellent communication & advocacy skills; great initiative; seek challenges, growth and change; and have a proven ability to actively collaborate, lead and deliver work to improve outcomes & experiences for your patients, team members and the organisation.

You will work closely with the Age and Complex Physiotherapy Grade 4, Clinical Lead who will provide you with clinical supervision and explore opportunities to develop your clinical, quality/research, and leadership skills. You will join the Aged and Complex Care Physiotherapy Leadership Team with oversight of stream clinical support, quality/research and professional development across the spectrum of care of older people.

About you

To be successful in this role you will possess:

  • Recognised Tertiary qualification in relevant discipline,

  • Current registration with Australian Physiotherapy Board,

  • Eligibility for Membership of Professional Association,

  • Relevant post graduate qualifications, or commencement thereof,

  • Extensive clinical experience in Sub-acute/GEM/Rehab Physiotherapy (community and/or ward based),

  • Experience in supervision of staff and students,

  • Experience in leadership of staff, including professional development and supervision,

  • Experience in clinical education of staff and students,

  • Excellent team integration skills,

  • Sound knowledge of community organisations and networks,

  • Excellent communication and organisational skills.
