The Diabetes Care Coordinator provides the Pacific peoples’ diabetes education and self-management services (DSME-DCIP) based at The Fono’s Henderson practice.  The position of Diabetes Care Coordinator provides culturally appropriate and relevant diabetes education and self-management support to Pacific people experiencing the health condition.  This position works in partnership with the GP, nurse and outreach services at The Fono and is an essential component of our FonoCare Integrated Model of Care.

The position is 40 hours per week based in Henderson.  It is a special opportunity and requires a versatile worker with great communication and organisational skills looking for an exciting new role.  You may be a new graduate or an experienced worker with or without nursing qualification.  What you will have an ability to engage with Pacific people, an ability to learn about diabetes self-management and a real wish to make a change for better health and social outcomes for Pacific families.

Key Requirements:

  • Have suitable health education qualification and/or experience
  • Good communicator and relationship manager
  • Be well organised and able to organise others
  • Be computer literate
  • Have a full Drivers Licence
  • Be able to work flexibly
  • A Pacific language capability may be an advantage
  • Value and live The Fono approach to working with community groups and families
