Applications are invited from suitably qualified medical practitioners eligible for registration in New South Wales for the above position within the Department of Central Coast Health at Home within, Central Coast Local Health District CCLHD.

Central Coast Health @ Home (CCH@Home) provides access, triage and linkage to community based health services for people in need of urgent care at home. This includes provision of care to people who are admitted to Hospital in The Home (HiTH) under a CCLHD Admitting Medical Officer (AMO).
The Visiting Medical Officer (VMO) for CCH@Home works as part of a team in providing care to people who are at risk of hospitalisation utilising both virtual care technology and face to face clinic appointments. This is a key role in the provision of care to people in the community and supports appropriate and timely communication between the District, NSW Ambulance, Emergency Department, Hospital Inpatient Services, Residential Aged Care Facilities, General Practitioners and other key partners in the delivery of admitted and non-admitted community based health care.

The Visiting Medical Officer will:

  • Ensure that CCH@Home provides high quality medical and health care services with safe, efficient and effective use of allocated resources, in line with the organisational objectives and the Clinical Services Plan
  • Develop and implement out of hospital clinical models of care consistent with best practice and quality improvement
  • Provide contemporary comprehensive specialist services for optimal patient outcomes,
  • Provide and support training to clinicians across multiple disciplines to achieve quality education and safe clinical practice.
  • Participate in 7 day a week roster including afternoon and weekend as required.
  • Participate in an evening and weekend on-call roster if required.
  • Provide daily support to the CCH@Home care team in the review, triage, and care provision to people referred to the service who are at risk of hospitalisation.
  • provide daily support, review and clinical intervention for people admitted into Hospital in the Home (HITH) under the care of a CCLHD Admitting Medical Officer (AMO) in Gosford and /or Wyong Clinic.
  • Facilitate at a clinical level, linkages with other services providers servicing the CCLHD population including primary care / general practice and the primary Health Network.


Applicants should be a Fellow of the Australasian College of Physicians or Fellow of the Royal Australian College of General Practitioners and / or other specialist recognition as provided for in the Visiting Medical Officer Determination.

Central Coast is comprised of the twin areas of Gosford and Wyong, positioned between Sydney and Newcastle, which are collectively expected to grow from the current approximate 300,000 to 450,000 population before 2030. The Local Health District has strong connections with both Royal North Shore Hospital and the University of Newcastle.

  1. Medical Practitioner registered or able to be registered in Australia, inclusive of an unrestricted or 19AB exempted provider number.
  2. Recognised specialist qualifications, Fellow of the Royal Australasian College of Physicians or Fellow of the Royal Australian College of General Practionersand / or other equivalent specialist recognition.
  3. Appropriate experience to practice as a Senior Medical Practitioner in General Medicine or General Practice .
  4. Ability to work constructively and effectively in a Multidisciplinary team environment, demonstrating excellent interpersonal and communications skills.
  5. Demonstrated willingness and ability to lead and supervise medical and surgical trainees, residents and medical students; Previous experience in teaching the management of the deteriorating patient would be advantageous.
  6. Demonstrated interest in quality improvement methodologies.
  7. Demonstrated commitment to Caring for the Coast vision, goals and strategies, with demonstrated behaviours which align to the NSW CORE Values and CCLHD Values and Behaviours charter.
