War Memorial Hospital (WMH) is a 3rd Schedule Aged Rehabilitation hospital, situated in Waverley, owned and operated by Uniting and part of the South Eastern Sydney Local Health District (SESLHD). Services are comprised of 24 hour inpatient care, Monday to Friday outpatient departments, several specialist community teams inclusive of Transitional Aged Care, Aged Care Assessment Team, Geriatric Flying Squad, Young Onset Dementia, 7 day a week day centre, 24 hour accommodation service for regional guests and the full range of hospital support services. This is an exciting professional opportunity for a CNS2 (as per individual) to join the specialist aged rehabilitation inpatient team at WMH. This role is the lead coordination of the patients' continuum of care from point of referral/admission, hospital stay through to discharge. The Clinical Care Coordinator (CCC) is the key contact for all external referrals to WMH. The CCC supports and engages the nursing team in providing optimal person-centred care with effective patient goal setting and management.

The CCC reports to the Nurse Unit Manager and is a valued member of the senior Nurse Leadership team comprising of the NUM, CNC and CNE alongside a committed nursing & multidisciplinary team. The CCC represents on various hospital committees and will be required to maintain a high level of communication internally with staff, patients and carers as well as external to the organisation inclusive of other Local Health District networks.

This position is a full-time permanent role. Uniting is a not-for-profit organisation with competitive salary packaging.

Selection Criteria:

  1. Registered Nurse with current Australian Health Practitioner Regulation Agency (AHPRA) registration, a minimum of 3 years full time equivalent post registration experience and in addition holds a post graduate qualification relevant to the clinical speciality e.g., Rehabilitation, Aged Care or other qualifications or equivalent experience deemed appropriate by WMH
  1. Demonstrated competence and advanced knowledge of clinical nursing assessment and treatment skills within Aged Rehabilitation speciality or similar.
  2. Demonstrated understanding of patient centred care and goal setting, with ability to advocate on behalf of the patient for high quality personalised clinical care.
  3. Demonstrated clinical nurse leadership skills which support and enable the professional voice of nursing colleagues within the MDT environment.
  4. Demonstrated experience and ability to facilitate complex discharges in collaboration with the MDT and to liaise with external agencies in relation to patient flow.
  5. Advanced interpersonal communication and networking skills inclusive of oral, written, and presentation skills across a range of audiences, with high level computer literacy in use of MS Office applications and other relevant health databases.
  6. Demonstrated understanding of quality improvement principles and frameworks including unit/service-based projects, initiatives and hospital accreditation processes e.g. National Standards and WHS Profile
  7. Understanding of clinical performance measures at unit level and their context at hospital level, e.g., clinical patient outcomes (FIM), length of stay, public/private patient mix etc.

All NSW Health workers are required to have received at least three doses of a COVID-19 vaccine AND an Influenza vaccine by 1st June 2023 or have an approved medical contraindication certificate.
