Nurse Practitioner Candidate - Hospital in the Home

Western Health is looking for a Nurse Practitioner Candidate with a passion for achieving great patient outcomes using evidence based practice focusing on the right care in the right place at the right time.

The role entails providing key advanced clinical nursing care and support to patients within the acute HITH program, working autonomously and collaboratively with the medical staff within Western Health, and the Hospital in the Home medical, nursing and allied health staff and external service providers.

The Nurse Practitioner will be responsible for maintaining their own clinical workload, working as an integral part of the Hospital in the Home team. With a focus on providing advanced clinical nursing care tailored to support the needs of acutely unwell patients in the community setting.

The Nurse Practitioner will provide evidence based, patient centred, autonomous and accountable care incorporating clinical practice at an advanced level including, but not limited to prescribing of medications, ordering diagnostic tests and referral to other services/clinics.

The role will also include providing advanced clinical support and leadership to nursing staff and an opportunity to engage in research and quality projects.

To be Successful in this Role, you will:

  • Be currently enrolled in an accredited Master’s program to achieve endorsement as a Nurse Practitioner with the NMBA
  • Have experience in relevant areas of practice including, geriatric care, chronic disease management and management of acute illness
  • Have a strong commitment to providing best care
  • Have highly developed organisational, critical thinking and assessment skills
  • Be committed to ensuring the Unit achieves all KPIs including workforce, quality, access and financial
