About the Role

We are seeking experienced health social workers to be available as part of our casual social work bank, tofill ad hoc and casual vacancies across all Eastern Health bed-based sites. This is an exciting opportunity, offering diversity and challenging clinical work. You should be skilled in risk assessments andcomplex discharge planning, have an ability to flex across clinical areasand aim to make a difference to patients, families and teams. You will connect quickly with new teams with your can-do attitude.

About You

We are looking for experienced Grade 2social workers whoarekeen towork withdynamic teams that promote best practiceand great patient outcomesin all clinical areas.

You will be working as part of multidisciplinary teams inacute and sub-acute programs. You will experience a supportive environment that ensures structured supervision, professional development opportunities and engagement in research and quality activities.

As the successful applicant you will have experience and competency in psychosocial assessments, risk assessments, grief and bereavement support, care planning, decision-making and discharge planning as well as liaison with internal and external key stakeholders. You will also assist in thedevelopment of services within the clinical stream by applying evidence based, and client centred care within a multi-disciplinary setting.

With a ‘can do’ attitude, flexibility, resilience, and excellent interpersonal skills, you will be a valued member of the Eastern Health Social Work team.

About the role

You will be responsible for:

  • Providing a clinical social work service to patients at Eastern Health
  • Conducting comprehensive clinical assessments (including risk assessments for vulnerablechildren and adults)
  • Providing support to colleagues
  • Supporting social work students
  • Undertakingquality projects
