About the Role:

An exciting opportunity is presented to work within the Northern Adelaide Local Health Network as a Physiotherapist within the Allied Health Intermediate Care Service (ICS). ICS comprises of Physiotherapy, Dietetics, Exercise Physiology, Occupational Therapy, Podiatry, Social Work and interdisciplinary complex self-management support services, operating as an intermediate service between hospital admission and community-based supports. Our team works collaboratively and in alignment with other services within NALHN Clinical Divisions as well as local Aboriginal Health Services, with a focus on optimising functional and health outcomes and reducing hospital admissions for clients with Diabetes, Chronic Cardiac and Respiratory Conditions, and vulnerable people with other chronic conditions. At ICS we aim to meet the needs of the client and “together, we connect with people living with chronic conditions to live the life they choose”.

The Physiotherapy team work predominantly with clients with Chronic Respiratory conditions, providing community based outpatient services inclusive of Pulmonary Rehabilitation and airway clearance.

The Physiotherapist (AHP2) is operationally responsible to the Senior Manager, Intermediate Care Services for the development, implementation, coordination and evaluation of quality Allied Health Chronic Disease Intermediate Care Services (within a self-management support framework) for patients of the Northern Adelaide Local Health Network (NALHN) and applies clinical experience, increasing clinical knowledge and professional competence to plan, implement and evaluate comprehensive and integrated services to the needs of clients of NALHN. The Physiotherapist (AHP2) works under limited direct clinical supervision and may provide direction and support to less experienced therapists, allied health assistants and students.

Working in conjunction with the multidisciplinary team and other health professionals the Clinician optimises patient functional and health outcomes in this specialty area.

The Physiotherapist works collaboratively with the ICS multi-disciplinary team and liaises directly with medical, nursing staff of NALHN and with other community agencies.

Educational/Vocational Qualifications

  • Appropriate Degree or equivalent qualification which entitles registration with the Physiotherapy Board of Australia.
