The St George Hospital and Health Services is part of the South Eastern Sydney Local Health District. It is an accredited, principal teaching hospital of the University of New South Wales and is not only the largest hospital within the Local Health District, but is among the leading centres for trauma and emergency management in NSW. St George Hospital is located 14kms from the Sydney CBD with easy road links to Sydney’s north, south and west. Just a 10 minute walk from Kogarah train station, St George Hospital is very easily accessible by public transport. Plan, coordinate, and implement in-depth levels of clinical expertise and extensive specialised knowledge within social work in order to provide high quality care to patients and clients of the St George Hospital consistent with NSW Health, South Eastern Sydney Local Health District (SESLHD) policies, procedures and standards.

The social worker will have responsibility for providing clinical care and care coordination to people across SESLHD who have end of life and palliative care needs who are in the last 12-24 months of life. The social worker will support these patients through the new Community Supportive Care Services (CSCS) model at St George Hospital (SGH).

This is a position suitable for clinicians interested in palliative and supportive care and has significant scope for role development. The position will assist with triage of patients to the appropriate community services. Once accepted to the CSCS, social worker will provide comprehensive health assessments, therapy, education and support to patients and their families.

Within this role, there will be a requirement to undertake clinical reviews across clinic, community and virtual care settings, either alone or in conjunction with a multidisciplinary team.

The successful candidate will be supported by the Supportive Care team at St George Hospital as well as by the social work team.

In addition to the above, there will be opportunities to lead education and research activities and to undertake strategic planning and capacity building activities for this exciting new service.

Employment of a temporary visa holder may only occur ifno suitable permanent resident or citizen of Australia has been identified for this position following suitable labour market testing.

All workers and new recruits are required to receive 2 doses of a Therapeutic Goods Administration approved or recognised COVID-19 vaccine to commence employment/ engagement or continue to work within a NSW Health service.

A worker and new recruit will be considered compliant if they have a medical contraindication to all available Therapeutic Goods Administration approved or recognised COVID-19 vaccines and provide medical contraindication evidence in line with the policy requirements.

In addition, all Category A workers and new recruits are required to receive one dose of the seasonal influenza vaccine annually to be considered compliant.

Category A workers and new recruits who are non-compliant with seasonal influenza vaccination or have a medical contraindication to influenza or COVID-19 vaccinations must comply with all other infection control risk reduction strategies as directed while working in a Category A position.

  1. Qualifications in accordance with Schedule C of the NSW Health Service Health Professionals (State) Award and eligibility for membership of the AASW.
  2. Comprehensive post graduate clinical experience in the provision of social work intervention with clients with end of life or palliative care needs
  3. Excellent negotiation, decision making and advocacy skills including the ability to interact constructively and collaboratively with a diverse range of stakeholders, throughout all organisational levels
  4. Ability to provide in-depth advice and education to enhance service delivery and to guide and support others in providing service excellence, with the capacity to provide clinical supervision to other staff
  5. Ability to work as part of a multi-disciplinary team and to apply advanced reasoning skills and independent professional judgement when dealing with situations of a complex or critical nature
  6. Ability to initiate, lead, complete and share quality improvement initiatives and service evaluation processes and lead ongoing quality improvement activities with other staff
  7. Demonstrated commitment to ongoing clinical and professional learning
  8. Current NSW Class C Drivers Licence or equivalent and ability to travel as required
