Enjoy the far North lifestyle while working with Transition Care Program Older Persons Sub-Acute Rehabilitation Service as Physiotherapist. In this role your will provide an effective and efficient physiotherapy service to older people, and their carers and families, who have been discharged from acute and sub-acute care settings and accepted into the Transition Care Program. This includes working collaboratively within a multidisciplinary team to case manage the wider needs of allocated clients.
