Title: Registered Nurse - Community

Location: North West Tasmania

Position Type: Permanent full time, day worker/shift worker position, working up to 76 hours per fortnight, commencing as soon as possible *hours can be negotiated with the successful applicant.

Salary: $80,524 to $109,175 per annum (pro rata) + 12% superannuation contribution.

The Position

Delivering a broad range of community-based services, including chronic disease management, wound care, continence support, health assessment, health promotion, palliative and supportive care, and care coordination to help people remain healthy and independent in their own homes and communities. Operating within a Primary Health Framework, the team deliver nursing and home care services to clients across the West Coast region both in home and in the clinic environments.

Service delivery models include:

• General Community Nursing
• Home Care Services
• Allied Health Clinics
• Emergency First Response Unit

Nursing care provided includes but is not limited to:

• Chronic and complex wound care including Negative Pressure
• Wound Therapy
• Management of venous access devices (PIVC & CVAD)
• Indwelling and suprapubic catheter management
• End of life palliative care including syringe driver management
• Post-acute care following discharge from hospital
• Medication management including support of medication administration via various routes and devices
• Chronic Disease Management
• Preventative Health

Community Health Nursing West Coast provides comprehensive, person-centred nursing care across the remote and rural communities of Rosebery, Queenstown, Zeehan and Strahan.
