Title: Clinical Nurse Consultant - Chronic Kidney Disease and Dialysis

Location: North Tasmania

Position Type: Permanent, part time, day worker position, working up to 48 hours per fortnight, commencing as soon as possible *hours can be negotiated with the successful applicant.

Salary: $118,297 to $125,543 per annum (pro rata) + 12% superannuation contribution.

The Position

Join the Department of Health as a Clinical Nurse Consultant – Chronic Kidney Disease and Dialysis, providing expert clinical leadership and education within a multidisciplinary renal team. In this rewarding role, you will support people living with chronic kidney disease (CKD) and their families through education, care coordination, and service development, improving patient outcomes and continuity of care across the renal care journey. Working collaboratively with nephrologists, nursing staff, and allied health professionals, you will lead CKD patient education initiatives, coordinate dialysis access referral pathways, and contribute to quality improvement, research, and evidence-based practice within the renal service.

Key Responsibilities:

  • Deliver a nurse-led chronic kidney disease education service.
  • Provide education and support to patients and their families regarding CKD management and treatment options.
  • Coordinate and develop referral pathways for dialysis vascular access.
  • Maintain and audit CKD patient databases to support service planning and outcome evaluation.
  • Monitor and report against key performance indicators.
  • Support evidence-based nursing practice through literature review and guideline development.
  • Contribute to quality improvement activities and nursing research initiatives.
  • Provide expert clinical leadership and specialist advice within the multidisciplinary renal team.
Health Service

The Renal Service provides specialist care to patients living with chronic kidney disease and those requiring dialysis therapies. The team is committed to delivering high-quality, patient-centred care and education, supporting individuals and families to make informed decisions about their treatment and ongoing health management. Renal Services - North (Tas) - has an Acute, in-hospital, renal unit in the Launceston General Hospital, and an outpatient Satellite Renal Unit, in the Launceston Community Health Centre, 22 McHugh Street, Kings Meadows - which provides Haemodialysis and Renal Home Therapies, as well as chronic kidney disease education and preparation for dialysis.

Essential Criteria
  • AHPRA Registration
  • Australian Working Rights
  • A minimum of 5 years' experience as a Registered Nurse with Renal Nursing experience.
Skills and Experience
  • To be able to provide clinical nursing consultancy on issues of clinical practice, involving Chronic Kidney Disease and Renal Supportive Care and act as a role model and expert clinician within Renal Services - North.
  • To be responsible for the effective planning, implementation and evaluation of a range of educational programs and activities for CKD patients and their families.
  • To be able to promote the maintenance of effective communication channels between the members of the multi-disciplinary team, to support the CKD - education pathway for patients with Chronic Kidney Disease.
  • Strong understanding of contemporary health education principles and chronic disease management.
  • Ability to lead quality improvement initiatives and contribute to research activities.
  • Excellent communication and interpersonal skills, with proven ability to work effectively in multidisciplinary teams.
