Justice Health and Forensic Mental Health Network is seeking an Integrated Care Coordinator (HM1) on a temporary full-time basis up to Mar 2027 for the Sydney Metro/MRRC area.

You will coordinate care for adult patients with chronic or complex health needs, support Network clinicians, assist with continuity of care, case management, transfer of care into the community and post-release follow up as required for custodial patients.
