Position details
The Team
The CNC Patient Flow sits within the Integrated Operations Centre as a part of the Patient Flow Team at the Launceston General Hospital and works collaboratively to ensure the right person receives the right care in the most appropriate setting.
The Role
The CNC provides expert advice and leadership within an interdisciplinary team providing pathway development, patient assessment, care coordination and implementation of planned transfer of care to District Hospitals and Private Hospitals from within the clinical settings of the Launceston General Hospital (LGH) to enable care needs to be met in the most appropriate environment. This is a shift working position working day shifts 7 days a week.
You will need
- Provide an advanced level of professional clinical expertise in care management for the patient transferring care in both the acute and Primary Health settings.
- Provide advice and support to other members of the interdisciplinary teams; to ensure continuity of care for clients with complex care coordination needs.
- Support the Nursing Director Integrated Operations Cantre and the interdisciplinary team in the promotion of the Patient Flow Unit Team, including developing and maintaining strong communication links with the Health Care Professionals within the Agency and other key groups and consumers regarding supporting the needs of clients, their families and carers.

Details of appointment
Permanent part time, shift worker position, working up to 40 hours per fortnight, 31 December 2026.
*notwithstanding hours to be negotiated with the successful applicant.

Salary
$118,297 to $125,543 per annum (Pro rata). Our Employer 12% superannuation contribution is on top of this amount.
- Salary range is in accordance withNurses and Midwives (Tasmania State Service) Agreement 2023
In addition, the following allowances calculated on the salaried incremental point may be available:
- Post Graduate Allowance of up to 10% based on qualification type (RN)

Eligibility
Successful applicants will be required to meet the essential criteria
- Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse
Applicants should note the following criteria are desirable:
- Holds or is working towards a relevant post graduate tertiary qualification
- Current Driver’s Licence.
