Berry Street Yooralla is seeking a Case Manager for the Multi-Agency Support Team (MAST) in Ballarat. You will coordinate practical, individualized support for young people aged 10-17 across Ballarat, Moorabool and Ararat, working with police, education and family services to close service gaps.

You will deliver trauma-informed, culturally safe case management, build protective factors, and connect youth and families with education and community resources in a collaborative setting.
