As a key member of the ATSICHS Brisbane team, the ATSICHS Kin Connector & Family Support Specialistprogramis a unique initiative that works toidentifykin placement pathways for children and young peopleresidingin the child safety residential OOHC system. The aim of the program is tore-unifyfamilyunder safe and thriving conditions. The program is not only excludedtokin placement options but also places a great emphasis on family reunification with biological parents, when safe to do so.Coordinatedand culturally safe supportand interventions are provided to families. By offering a comprehensive mix of services tailored to family and community needs, we aim to build family capacity, strengthen community connections, and safeguard the wellbeing of children.

Duties

  • Effectively engage clients(extended family and kin of child)through cold-call home-based visits. Turn initial 'kin engagement'home visitintoongoingconsentand interest in becoming a Kin Carer.
  • While completingfamilyengagementactivities,ask toidentifyotherstrong family members that couldpossibly becomea kin carer. Actively seekcontactdetails and relationship connection with child and broader family.
  • Engage the child and/or lead Residential Care Worker indeterminingthe views andwishes of family placement options with family, asdirected by line Manager. Utilise the Narrative Assessment Tools to assessbarriers and wishes.
  • Assistin the assessment of carers, including initial screening, Blue Card submission, and PAC assessments.Advocate and support families when being assessed externally.
  • Develop a comprehensive Transition Plan(Case Plan)that explicitly addresses allconcernsoutlined in the (1) referral, (2) program assessment tool, (3)TransitionNAT tools, (4)Child Assessment,and(5) Carer Assessment.
  • Deliver case managed services according to the program model of care, ensuringminimum case load requirements aremaintainedto ensureequitableaccess to services for community.
  • Maintainaccurateand detailed case notes of all client interactions, ensuring case notes are written from the perspective of clients in their voice.
  • Ensure clients completethe 'Have Your Say' client feedback tool andsubmit. Act on the client feedback to make continuous improvements to your practice and approach with clients.
  • As perMOBnetPerformance module,participatein monthly prescheduledsupervision with direct lineManager.Ensure Supervision tasks are thepriorityin work completion.Ensure all associated monthly supervision tasks inMOBnetarecompleted (two-way feedback, team culture scoreetc).

Desired Skills and Experience

Required

Bachelor's degreein a relevant social services field withat least 4 yearscase managementexperiencein family support, child protection, youth support,housingor related field.

OR

Diplomain relevant Community/Services fieldwithat least 6 years in a case management rolein family support, child protection, youth support,housingor related field.

OR

At least6 years case managementexperiencein family support, child protection, youth,housingor related field. Acommitment tocomplete Diploma(funded by ATSICHS) while working.Andidentify asAboriginal and/or Torres Strait Islander(with confirmationproof)ORnon-indigenous and having worked for an Aboriginal Community Controlled Organisation forat least 4 years.

AND

  • Exceptional case management skills, including grammatical writing ability, personal organisation and time management, and execution of services according to the model of care phases of work.
  • Demonstrated ability to create new social work theoretical frameworks of practice, aligned with ATSICHS values. Ability to coach other staff on client interventions that build client motivation, insight and acceptance, self-awareness, decision making (agency), and sustainable behaviour change.
  • Ability to apply technical knowledge of relevant legislation (child protection/housing/youth justice etc). Ability to critique/ and problem solve legislative grey zones, while managing organisational and child/client harm risks.
  • Influence decisions of external stakeholders, including Child Safety, Youth Justice, Schools, and Hospitals. Whole-of-system Internal Integration: Ability to connect through systems panel coordination numerous internal ATSICHS programs and services to activate wrap-around care for clients, trading-off and negotiating resources across the organisation and between other teams to achieve client impact.

Additional Requirements

  • Current Working with Children's Card (Blue Card) or be eligible toapply.
  • National Police Certificate.
  • Driver's License.
  • Evidence of Work Rights.
  • Travel to work in the community on a regular basis.
  • Able to work after-hours and on weekends.
