Join the Hospital in the Home (HITH) Service at Cumberland Hospital, where you'll be part of a progressive healthcare team providing acute and sub-acute care beyond traditional hospital walls. Our service enables patients to receive safe, evidence-based treatment in the comfort of their own homes while maintaining strong clinical oversight and support.

What you'll be doing
As a Registered Nurse within the Hospital in the Home service, you will provide evidence-informed nursing care that promotes positive health outcomes and supports patients throughout their recovery journey. Working independently and collaboratively, you will assess, plan, implement and evaluate care while partnering with patients, carers and healthcare professionals.

·Deliver comprehensive nursing assessments and develop individualised care plans

·Monitor, implement and evaluate clinical interventions to achieve optimal outcomes

·Collaborate with patients, carers and multidisciplinary teams to coordinate care

·Promote evidence-based practice, patient safety and continuous quality improvement

·Support clinical learning, knowledge sharing and the development of nursing colleagues

Essential Criteria:

·Registration: Currently authority to Practice as a Registered Nurse with the Australian Health Practitioners Regulation Agency (APHRA)


Selection Criteria:

1.Current unrestricted Drivers License

2.Recent relevant experience in clinical setting as deemed appropriate by WSLHD

3.Excellent time management and organisation skills with attention to detail

4.Evidence of computer literacy and understanding of relevant health related information systems

5.Effective interpersonal skills with a demonstrated ability to develop and maintain relationships with clients, carers and the wider health care team
