  • It is a community health centre based position requiring travel across sites and into private residences.

  • This position is required to participate in afternoon, weekend and public holiday shifts.

  • The rotation to these shifts is minimal at this current time, and is subject to change at anytime.

This position provides care in the community to client's requiring, wound management, continence and catheter management, and supportive and palliative care, including end of life.

What you'll be doing
As a Chronic & Complex Community Registered Nurse, you will provide comprehensive nursing assessment, care coordination and clinical interventions for clients with ongoing and complex healthcare needs. Working both independently and collaboratively, you will play a key role in supporting clients and their families through proactive care planning, education and clinical management.

·Conduct comprehensive nursing assessments and develop individualised care plans

·Coordinate and deliver evidence-based clinical interventions, monitoring and ongoing care

·Partner with clients, carers and families to support health goals and promote self-management

·Collaborate with multidisciplinary teams to ensure seamless, integrated care and timely discharge planning

·Maintain accurate clinical documentation, participate in case reviews and contribute to quality improvement initiatives

Essential Criteria:

  • Registration: Current Registered Nurse division 1 Australian Health Practitioner Regulation Agency (AHPRA)
