About the role:
We have an exciting opportunity to work in a dynamic team leading our approach to the provision of quality care to clients of the Transitional Aged Care Program (TACP). This position will provide nursing care under the guidance of the Registered Nurse to patients referred to TACP. This position will also assist in the delivery of therapy and support services for clients of the TACP, working with a large multidisciplinary team to provide slow stream rehabilitation to clients who have recently discharged from hospital.

The tasks the AIN may undertake include: assisting clients with their Home Exercise Programs (HEP), escorting/community access, showering and bathing clients, and light domestic assistance.

The incumbent will document all client responses to direct care given, including progress notes and clinical records in accordance with the plan of care and organisational procedures. This position is Monday- Friday, with the occasional weekend or public holiday required.
TACP provides a package of services for up to twelve weeks duration to clients following discharge from hospital and who have potential to manage at home with rehabilitation and support services. The purpose of this intervention is to improve the client’s physical function, avoid further hospital presentations and to maintain their daily living skills. The AIN will support the service’s clients with personal care, transport, support rehabilitation goals and other activities as delegated by the multidisciplinary team.

Selection Criteria:

  • Personal integrity and demonstrated commitment to the Philosophy, Mission and Values of Mary Aikenhead Ministries and St Vincent’s Health Australia.

  • Relevant Certificate III qualification or have completed one (1) or more year training of a Bachelor of Nursing degree.

  • Current unrestricted NSW drivers license - Class C or above.

  • Demonstrated ability to work independently, under delegation, and the ability to collaborate within an interdisciplinary team.

  • Demonstrated effective interpersonal, written and verbal communication skills.

  • Demonstrated computer literacy skills and knowledge of an electronic client record system.

  • Demonstrated experience working with older people in a community/rehabilitation setting.
