Your Role as Care Partner (Virtual)
The Virtual Care Partner is responsible for delivering care management services to Support at Home customers to ensure they receive high-quality, person-centred care that meets their individual needs, goals, and preferences. The Virtual Care Partner works collaboratively with customers, their registered supporters, carers, and other health professionals to develop and implement continuity of care across effective care plans, coordination of service delivery, and monitoring outcomes in accordance with the Aged Care Act 2024 and Strengthened Aged Care Quality Standards.
This role provides care continuity for Community Aged Care stakeholders through the use of telephone communication as a primary source of contact. You’ll empower individuals to live more independently, safely, and meaningfully by:
- Coordinating personalised services that align with each customer’s goals, preferences and approved care plans.
- Managing customer budgets responsibly to ensure services are delivered within funding parameters and support long-term independence.
- Monitoring changing needs and emerging risks, responding proactively to maintain safety, wellbeing and continuity of care.
- Building trusted relationships through regular communication with customers, carers and supporters, ensuring their voices are heard and respected.
- Applying wellness and reablement approaches to encourage independence, confidence and meaningful daily living.
- Ensuring care plans reflect cultural, spiritual and diverse needs, including engaging with culturally specific organisations when required.
- Supporting informed decision-making, including empowering customers to take positive risks that enhance quality of life.
- Maintaining high-quality documentation and compliance, ensuring safe, seamless, and consistent care across all interactions.
What We’re Looking For
- A Certificate III in Individual Support (Ageing), Certificate IV in Aged Care, or equivalent qualification to help you confidently support customer needs
- Bring at least 12 months experience in aged care, disability services, or community services, using your practical skills to make a meaningful impact
- Use your digital confidence and experience with client management systems to document care accurately and support smooth service delivery
- Use your strong communication and relationship-building skills to collaborate with customers, families and health professionals
- Work confidently both independently and in a supportive team, using your time-management skills to stay organised and meet deadlines.
- Bring your passion for person-centred care, supporting customers to make informed choices that enhance their independence and wellbeing.
A Valuable Addition, Yet Not a Must-Have!
- Experience in call centre, customer service or broader healthcare settings is highly valued.
- Ability to use data and insights to support quality service delivery and performance.
- Additional qualifications in aged care or community services are an advantage.
- An understanding of wellness, reablement and working effectively with diverse communities.
- Familiarity with quality standards, continuous improvement and aged care regulatory frameworks.

Your Role as Care Partner (Virtual)

The Team:
The Community Forensic Mental Health Service is seeking a dedicated psychologist to join our specialist team providing psychological input to individuals involved in the criminal justice system. This is a great opportunity to be part of Tasmania’s only community based forensic mental health team, delivering evidence-based care, risk assessments, and professional consultation to a wide range of stakeholders.
The Role:
In this role, you will:
- Conduct comprehensive psychological assessments, including for risk (e.g. physical violence, sexual offending, stalking, etc.), psychological functioning, personality, and cognitive abilities.
- Provide court-ordered psychological reports and expert opinions
- Deliver consultation and support to broader multidisciplinary mental health services and justice partners.
- Work collaboratively with a skilled and supportive statewide team.

- Holds clinical or forensic endorsement (preferred but not required)
- Has strong assessment, formulation, and report writing skills.
- Enjoys working in multidisciplinary teams and values professional collaboration.
- High level consultation, liaison and negotiation skills including experience in developing collaborative working relationships with other service providers to ensure best client outcome.

This position has a primary focus of working with children and young people and providing specialised counselling to children, young people who are experiencing impacts from parental separation and high conflict including domestic and family violence.
You will provide a range of high-quality age-appropriate interventions including assessment; individual and group counselling; and psycho-education and therapeutic supports to children and young people (under 18 years) who are experiencing difficulties related to family relationships and changes
in family structure. The position also plays a key role in connecting children and their families with other relevant services through effective referrals and collaborations.
What makes a difference for us?
- Tertiary qualifications (minimum Bachelor) in a relevant human services field, with membership or eligible membership of an appropriate professional body (e.g. AASW, APS, PACFA Member Association) preferred and significant relevant experience
- Excellent counselling, therapeutic and interpersonal skills including knowledge of evidence based therapeutic interventions

About the opportunity
The position provides care to adults in centre or home-based appointments.

Your part in the Blue Care Team
Our dynamic retirement livings team at Argyle Gardens Retirement Village in Bundaberg is looking for committed and passionate Retirement Living Assistants to join their dedicated and close-knit team on a part time basis.
Blue Care operate over 32 retirement villages across Queensland, with more than 3,300 residents choosing to make a Blue Care communities their home.
Reporting to the Village Manager, the role will see you provide a professional working relationship with the Village residents and team members. As well as being the first point of contact for Emergency Response as part of the overnight on-call services. Your responsibilities will include:
- Observe and report any changes or concerns in resident’s health, well-being or behaviour to Village Management.
- Assisting with meal service and dining room set up.
- Manage emergency response procedures, including medical attention as required, is provided and documented in accordance with RVA accreditation guidelines, UnitingCare contracts and resident contracts.
- Respond to resident alarms, applying basic first aid where required and informing/engaging the appropriate authorities.
- Ensure Community buildings and facilities are secured in accordance with set procedures.
- Report on site maintenance or equipment issues to relevant staff.
- Act as a team player and foster a safe work environment for self and others.
What makes a difference for us?
- Previous experience working in hospitality service delivery.
- Excellent attention to detail
- Demonstrated experience in responding to Workplace Health and Safety emergency procedures
- Current first aid certificate and CPR certificate
- Experience working in a multidisciplinary team.
- Experience in retirement living, senior’s housing or aligned sector is highly regarded.
