Responsibilities
- Develops and implements individual treatment plans for students in need of diabetes care.
- Develops and presents lessons in diabetes management, in close coordination with paraprofessionals to benefit students in the least restrictive environment, including providing hands on-training, materials, and protocols to paraprofessionals.
- Provides annual and refresher training as well as student specific training to nurses, paraprofessionals, and other identified staff.
- Reviews medical and health records of students receiving diabetes care for completion of data.
- Works with regional supervisors to identify nurses and agency covered schools without a regular nurse to provide support and training in diabetes care and devise appropriate interventions.
- Develops relationships with insulin and technology representatives to facilitate diabetes training and support to nurses and school staff.
- Develops relationships with diabetes providers in their areas to serve as a liaison between providers, families, and schools, as needed.
- Participates in multidisciplinary and parent conferences to discuss student progress and offers specific recommendations for diabetes health programs related to individuals and groups of students.
- Assists in developing section 504 plans, promotes collaboration between Section 504 Coordinators, and serves as a resource for questions related to section 504 or IEP services.
- Instructs and consults with teaching and supervisory staff on matters relating to diabetes services.
- Prepares training opportunities for OSH nurses in all aspects of diabetes care and maintains database of training and implementation goals.
- Functions as preceptor for nursing students from accredited nursing programs in local colleges and universities.
- Contacts primary care providers and/or endocrinologists regarding students' diabetes management related to the continuity of care both in school and at home.

Qualification Requirements: Minimum
- A valid New York State Registered Nurse License.

About the roleYou'll help shape how OT lives inside the clinic - from referral pathways and group program design, to how OT FCAs and therapy are introduced to families already engaged with the speech and psych teams. You won't be doing this alone. You'll work closely with the Clinic Director and have external supervision from a senior OT, calibrated to your level and goals.
By year two, as the caseload grows, you'll begin supervising junior OTs and have an increasing say in how the team is built.

Our Medicare Urgent Care Clinic in Warrnambool is looking for a highly professional, friendly, well organised Nurse Practitioner to join our clinical team on a sessional/casual basis as part of our Triage Team.
About the RoleThe objectives of the Nurse Practitioner (NP) role is to provide safe, efficient and quality care to patients and their families presenting to the Medicare Urgent Care Clinic (UCC) and to demonstrate clinical and professional leadership as a senior staff member. The NP is an integral member of the UCC health care team who practices in collaboration with other health professionals to assess and manage patients within their clinical context using nursing knowledge and skills within a fast-paced environment. The NP contributes towards the Medicare UCC program, Epichealth and community through involvement in education, research, quality improvement and professional leadership.
Key responsibilities:- Provide advanced and autonomous expert care to patients and families in accordance with Nurse Practitioner legislation and functions by: Prescribing medications, Ordering and analysing pathology and radiological tests, Referring patients to GPs and other health care professionals whilst working as part of our Triage Nurse Team.
- Work collaboratively to support medical and nursing colleagues in the performance of their clinical role.
- Actively communicate and coordinate with colleagues and specialist services when providing team care.
- Record and maintain accurate, adequate and relevant patient records using the established practice protocols and computerised medical rec ords system (Best Practice) while meeting legislative standards and accreditation requirements.
- Provide leadership in the coordination and implementation of quality improvement activities specific to the Nurse Practitioner role.
- Ensure nursing compliance with Federal/State legislation, nursing competency standards and Epichealth policies and procedures.
We are seeking someone who has:
- AHPRA registration as a Registered Nurse with endorsement as a Nurse Practitioner.
- Advanced clinical experience in the assessment and treatment of illness and injury and health promotion relevant to the role and within individual scope of practice.
- Demonstrated extensive professional and/or leadership experience.
- Demonstrated clinical problem-solving skills including conceptual and analytical ability.
- Advanced interpersonal, communication and negotiation skills (both written and verbal).
- Knowledge, understanding and experience in quality improvement, its practical application in meeting patient's needs and quality service delivery.
- Knowledge, understanding and experience of research findings to support evidence-based practice.
- Experience in quality improvement, its practical application in meeting patients' needs and its relationship to strategic development.
- Sound knowledge of relevant legislation and health regulating Acts and legislative obligations.
- Evidence of appropriate and continuing professional indemnity insurance.
- Demonstrated understanding of key cultural issues influencing practice, patient relationship and patient need.
- Experience within a general practice/emergency care setting.
- Experience with the accreditation process and quality improvement requirements.
- An understanding or experience of the RACGP Standards for General Practices (5 th Edition).
- Appropriate Tertiary qualification ie. Associate or Bachelor's Degree in Nursing.
- Basic Life Support certification.
- Advanced Cardiac Life Support certification.
- Triage certification.
- A current National Police Records Check.
- A current Working with Children Check.
- Working knowledge of Microsoft Office software systems eg. Word and Excel.
- Current Driver's Licence.
