About the Role:Under the Strengthening Medicare Initiative, Northern Health are piloting an integrated outreach model to deliver rapid care and support to older persons in the community. The aim of the Older Persons Care Hub (OPCH)is to provide non-admitted specialist geriatric assessments and rapid short-term interventions to older people in their homes who are at high-risk of hospital presentations.
The OPCH Team Leader will oversee the development of the OPCH, the coordination of the clinical team and the delivery of patient care. This role is also responsible for the development and implementation of the OPCH project, engaging key stakeholders, ensuring key deliverables are achieved and providing regular progress reports to the Department of Health. This is a leadership role that will be responsible for the performance, practice and professional behaviour of the multidisciplinary team (in liaison with relevant Allied Discipline leadership) and professional supervision of the Nursing team. The Team Leader will also be required to carry a clinical caseload (phone or home based).
The OPCH is currently a 6 day per week service working towards a 7day a week service (Hours of work 8.00am - 4.30pm). The successful incumbent will need to be flexible with their working hours which will include working on weekends and public holidays. This is a 40 hour a week position and is fixed term to June 30th 2028.
To be successful in this role you will have:- Minimum 7 years clinical experience
- Extensive experience in the management of complex clients in the acute/community healthcare setting
- Expert knowledge of the broad health care sector
- Proven experience coordinating a service or managing a clinical team (ANUM or above)

Job Description General Description
The primary responsibilities of a SCRA include managing and overseeing clinical trials, delivering high-quality data within established timelines and following monitoring procedures compliant with GCP, ICH guidelines, local regulations, and standard operating procedures (SOPs). - Monitor clinical trial sites to ensure compliance with the study protocol, ICH-GCP, and regulatory requirements.
- Perform Pre-Study Site Selection, initiation, routine monitoring, co-monitoring and close-out visits.
- Verify the accuracy, completeness, and integrity of case report forms (CRFs) and source documents.
- Identify and resolve site issues and provide appropriate training to site staff.
- Evaluates the quality and integrity of site practices, escalating quality issues as appropriate.
- Ensure proper documentation of study activities and timely data collection.
- Ensure study milestones are met as planned for assigned sites
- Maintain regular communication with investigators, site staff, and internal project teams.
- Assist with regulatory submissions, ethics submissions, and site contract negotiations as needed.
- Prepare monitoring visit reports and follow-up letters in a timely manner and as per applicable SOPs.
- Support audit readiness and participate in sponsor or regulatory inspections when necessary.
- Manages site performance by tracking recruitment and enrolment, case report form (CRF) completion, and data query resolution.

This is a full-time fixed-term (12 months) position. While flexible and hybrid working arrangements are supported, the successful applicant will be required to maintain a regular presence at our Blackburn North Business Centre office in line with Ambulance Victoria's hybrid working model.
About the roleReporting to the National Standards Accreditation Lead responsibilities of this position include but are not limited to:
- Provide informed advice and support to operational leaders on quality, safety, performance and improvement priorities, drawing on risk, data, patient experience and evidence
- Contribute to quality planning by collating and applying patient experience, risk, performance data and evidence to inform planning activities within assigned work areas
- Support the delivery of quality assurance activities and organisational readiness for regulatory and accreditation requirements, including the NSQHS Standards, to promote safe, effective, person-centred and connected care
- Build and maintain effective working relationships with operational leaders, stakeholders and governance forums to support the delivery of Best Care priorities
The successful candidate will hold a degree or relevant industry certification, supported by demonstrated relevant experience, or an equivalent combination of training and professional experience. Certifications and/or experience in Improvement Advisory/Coaching, Project Management, Change Management, Clinical Governance, or Advanced Data for Improvement will be highly regarded. Other requirements include but not limited to:
- Analysing and interpreting data to identify trends, insights and opportunities for improvement
- Creating clear, meaningful reports, dashboards and visualisations to support decision-making
- Supporting quality, safety and performance improvement initiatives using recognised improvement methodologiesSupporting the application of clinical governance frameworks and compliance with relevant standards, including the NSQHS Standards
- Building effective relationships with stakeholders and providing clear, evidence-informed advice to support improvement initiatives and organisational outcomes
