About the role
The Homeless Outreach Team (HOT) is a community-based, multidisciplinary service dedicated to supporting people who are experiencing homelessness or are at risk of homelessness.
Lead one of four primary Homeless Health Teams within HOT, providing nursing leadership and clinical oversight, with overall responsibility for budget and human resource management, while maintaining a strong focus on patient flow, patient care and service delivery.
Lead a community-based, multidisciplinary service dedicated to improving access to healthcare for people experiencing homelessness or at risk of homelessness through a person-centred, trauma-informed and harm-minimisation approach.
Work collaboratively with local health providers, housing services, government agencies and non-government organisations to deliver assessment, treatment, education, support, advocacy, referral and care coordination, and assisting clients to obtain and maintain safe and stable housing.
Oversee intake and referrals to the Homeless Health Service (HHS) through a "no wrong door" approach, ensuring people are connected to the right services regardless of how they enter the system.
Oversee daily health outreach clinics across multiple locations throughout the city, as well as street outreach, in partnership with government and community homelessness services, bringing healthcare directly to those who may otherwise face barriers to access.
Deliver brief interventions, health assessments, treatment, referral pathways and care coordination to support individuals in accessing mainstream health services of their choice.
About you
Currently registered as a Registered Nurse (Division 1) with the Australian Health Professionals Regulation Agency (AHPRA)
Postgraduate tertiary qualification in a clinical specialty or management or a commitment to undertake postgraduate studies within 12 months; equivalent experience may be considered.
Demonstrated operational, financial, human resource and workplace health and safety management experience at a departmental level, with highly developed communication and interpersonal skills.
Comprehensive understanding of clinical governance and current nursing issues and demonstrated experience leading quality improvement initiatives, including the development, implementation, evaluation, and review of clinical policies, procedures, and service improvement strategies.
Proven ability to lead and support the delivery of evidence-based, patient-centred care within a complex healthcare environment, integrating theoretical knowledge, clinical skills, critical thinking and analysis into daily practice.

About the Department
10 East Neurosciences is a 24 bed unit comprising of 24 Neurosurgical beds. We care for patients with higher dependency and complex care needs within our supportive multidisciplinary team. We strive to provide high quality patient focused care. The Unit cares for post operative Neurosurgical patients who have undergone brain, spine or epilepsy surgery. Our team is dedicated to developing and growing our staff so that everyone has the opportunity to reach their full potential. We encourage staff to pursue post graduate qualifications in Neurosurgery Nursing through the Australian Catholic University. Teamwork is at the core of everything we do.
About the Role
The Grade 2 Registered Nurse, under the guidance of the Nurse Unit Manager or Associate Nurse Unit Manager, will be responsible for the delivery of expert clinical care in his/her specific area. The Grade 2 Registered Nurse will be responsible for ensuring his/her own professional development in order to maintain clinical skills and knowledge.
Classifications for this position will range from YP3 - YP9 ($69,253.60 - $92,601.60 pro rata per annum).
Duties
- Participate in mentor / preceptorship programs
- Participate in Unit meetings and orientate new staff as directed
- Participate in ongoing education by attend training and study days as required or requested
- Work a rotating roster on night shifts only.

About the role
Deliver high-quality specialist assessment, psychosocial interventions, therapeutic sessions, and health education to clients who use methamphetamine, cocaine, and MDMA, including facilitating individual and group programs.
Develop, evaluate, and revise individualised multidisciplinary care plans and clinical pathways, incorporating discharge planning, routine outcome measures, and consultation with clients to ensure effective treatment.
Liaise with internal and external agencies, provide referrals, and coordinate with relevant services to ensure clients receive holistic, high-quality care, including bio-psycho-social assessments in line with best practice principles.
Participate in clinical review, supervision, professional development, and reflective practice, including training, peer feedback, and use of audio/video recordings to support skill development and ongoing learning.

LOCAL WORK ENVIRONMENT
This position is part of the Health Independence Program (HIP). The Access, Rehab and Clinics Stream consists of multidisciplinary intake unit, outpatient rehabilitation programs and Specialist Clinics including the Continence Clinic which is currently based at Kew and Fitzroy. The Continence Clinic provides specialist assessment and management of both urinary and faecal incontinence to people over 16 years of age in the geographical areas of the Cities of Darebin, Yarra and Boroondara and Melbourne.
This position may be required to work from any of the SVHM HIP sites and may be required to work within any of the HIP programs. This will be at the discretion of the HIP manager and will take into consideration appropriate skills and training.
POSITION PURPOSE
The Continence Physiotherapist will coordinate the specialist assessment and management strategies for clients referred with continence problems including pelvic floor rehabilitation and pain. Educational, supervisory responsibilities, evaluation and quality improvement activities will also be undertaken.
POSITION DUTIES
Provide highly demonstrable specialist assessment and management interventions for both urinary and faecal incontinence and associated disorders.
Complete comprehensive and evidence based assessments, care planning and treatment especially pelvic floor rehabilitation for outpatient clients who are experiencing continence difficulties.
Conduct pelvic floor group program including evaluation
Evaluate treatment /management using appropriate objective measures
Participate in triage of referrals as required
Participate in patient goal setting at multidisciplinary case review meetings.
Provide specialist education and advice to colleagues and clients regarding continence management and pelvic floor rehabilitation
Manage the discharge process for clients
Work in close liaison with colleagues, other key service providers, families and careers relevant to caseload
Establish and maintain effective working relationships within the Continence team, HIP, referral sources and other agencies
Act as a key resource person within the Continence Service and HIP
Communicate client issues effectively with other team members
Identify opportunities for improvement of service delivery and implement approved strategies to address these.
Contribute to the ongoing development and updating of current departmental clinical literature and resources as requested.
Participate in project working groups / quality improvement initiatives to improve the service
Achieve activity targets and complete activity statistics by end of working week or 3rd day of next month (whichever comes first)
Complete medical record documentation to required standards and on the day of intervention
SELECTION CRITERIA
- Minimum of 7 years physiotherapy experience
- Knowledge of relevant continence specific community resources and services.
- Demonstrated sound knowledge of current evidence based practices in the area of continence assessment, management and evaluation.
- Ability to assess and treat incontinence including undertaking vaginal and rectal examinations in women and rectal examinations in men, as appropriate.
- Ability to provide electrical stimulation and biofeedback training as appropriate in women and men.
- Ability to plan and carry out evaluative research in physiotherapy and continence practice at the level of beginning researcher.
- Demonstrated ability to work alone or within an interdisciplinary team.
- Highly developed organisational, planning and problem solving skills.
- Highly developed interpersonal skills including excellent communication and leadership skills.
- Highly developed documentation and computer skills with Microsoft office, Microsoft Word and Excel and

About the Role

The Clinical Research Coordinator/Registered Nurse (Clinical Research) is responsible for the coordination and delivery of high-quality, patient-centred clinical trials within The Kinghorn Cancer Centre.

About The Role
The Mater Hospital has an innovative and progressive Orthopaedic department with some surgeries utilising Robotics. There are 3 orthopaedic wards, including our rehabilitation ward. We care for patients following knee and hip replacements, shoulder, spine and foot surgery as well as sports injuries.
Our experienced team of health care professionals, care givers and support staff are equipped to provide the highest level of therapeutic, medical and surgical services.
We are one of Australia's leading orthopaedic hospitals and the only Australian hospital to be admitted into the International Society of Orthopaedic Centres.
About You
Essential Criteria:
Current AHPRA registration.
Bachelor’s in nursing
A minimum of 2 years’ experience in Orthopaedic Rehabilitation nursing.
Demonstrated ability to evaluate and deliver quality patient care.
Demonstrated commitment to professional development.
Desirable:
Previous Orthopaedic or rehabilitation nursing experience.

Things that work for us:
Broad shift availability is desirable and experience in Dementia care would be an advantage
Nursing Registration with the Australian Health Practitioner Regulation Agency is essential
Clinical nursing practice experience in an aged care facility
