The Eyre and Far North Local Health Network (EFNLHN) is seeking an Advanced Clinical Practitioner - Regional Complex Care to lead the delivery of high-quality, specialised acute care and disability services across our regional health network.
This newly developed position offers an exciting opportunity to provide expert clinical leadership, complex case management, strategic advice and service development across acute, subacute, aged care and community settings. Working closely with the Manager Network Operations, Patient Flow Coordinator and multidisciplinary teams, you will play a key role in supporting patient flow, complex discharge planning and better health and functional outcomes for consumers.
Working with a high degree of autonomy, you will use advanced clinical reasoning and contemporary specialist knowledge to support consumers with complex and often challenging care needs. You will also contribute to workforce capability, education, research, quality improvement and the continued development of best-practice models of care across EFNLHN and SA Health.
This is more than an advanced clinical role. This is an opportunity to shape the future of complex care in regional South Australia, while making a meaningful difference to consumers, families, carers and the communities we serve.
Key Responsibilities:
- Lead and provide expert specialised clinical services for consumers with complex care, disability, rehabilitation and functional needs across EFNLHN acute services.
- Provide advanced clinical leadership and consultancy to Allied Health, medical, nursing and multidisciplinary teams, supporting complex assessment, intervention, risk management and discharge planning.
- Drive effective patient flow and care coordination, working with consumers, families, carers, NDIA, disability and aged care providers and other stakeholders to develop safe and sustainable care pathways.
- Lead service development, education, quality improvement and research, translating contemporary evidence into clinical practice, workforce capability and innovative models of care.
- Provide strategic advice to senior leadership and contribute to regional and statewide initiatives, using clinical expertise, data and consumer outcomes to inform service planning and decision-making.

Essential Qualifications
Must hold a recognised allied health qualification within the Occupational Therapy profession and fulfill all requirements to achieve and maintain current practicing registration with AHPRA.

What you'll be doing
The Clinical Nurse Specialist Aged Care, using best practice evidence provides person-centered specialist nursing care at an advanced practice level for residents in the aged care setting in collaboration with individual/s and the multidisciplinary health care team. Provides clinical leadership, support and supervision for less experienced clinicians to achieve people focused quality care.
The Clinical Nurse Specialist – Aged Care will serve as the onsite Infection Prevention and Control (IPC) lead for the residential aged care unit, supporting compliance with the Aged Care Quality Standards and relevant legislative requirements. The role provides leadership in the implementation and ongoing management of an effective IPC program and providing IPC education and training to staff.
Selection Criteria
- Current Registration with the Australian Health Practitioners Regulation Agency (AHPRA) as a Registered Nurse and relevant post-registration qualifications in Aged Care with a minimum of three years' experience in the care of older people in the inpatient or residential aged care setting.
- Relevant qualifications in Infection prevention and control or willingness to obtain and a demonstrated understanding of antimicrobial resistance and stewardship.
- Demonstrated ability to undertake comprehensive assessments and develop a plan of care, in collaboration with the multidisciplinary team, which is focused on the expressed needs/wishes of the older person.
- Demonstrate ability to exercise extended autonomy in clinical decision-making capabilities and leadership in the development of nursing specialty practice advancement.
- Proven skills in case management and liaison and the ability to lead a case conference in order to facilitate a timely and appropriate care within the residential aged care setting.
- Demonstrated effective clinical communication through the use of information technology, written and verbal means and sound interpersonal skills with the capacity to interact with others in a personable and professional manner.
- Demonstrated knowledge and application of evidence-based practice and critical thinking to advocate for and implement individualised person-centred care at an extended role level with a commitment to quality improvement.
- Evidence of participation in own professional development, a commitment to maintaining competence and enhancement of skills with a willingness to apply and share professional expertise as a resource person and mentor others within the specialty.

About the Opportunity
An exciting opportunity is available for a Registered Nurse (Mental Health) to join the Newcastle or Lake Macquarie Community Older Persons Mental Health Service (OPMHS).The Community OPMHS is a specialist service providing assessment, treatment and ongoing care to older people experiencing mental illness, cognitive concerns and complex psychosocial needs.
We have two opportunities available, one at Lake Macquarie Community OPMHS is based at Charlestown Health Campus and provides specialist community mental health care across the Lake Macquarie area. Newcastle Community OPMHS is based at Watt Street, Newcastle, on the former James Fletcher Hospital site and provides specialist community mental health care across Newcastle and surrounding areas.
Both services are multidisciplinary teams comprising nursing, medical, psychology, social work and occupational therapy clinicians. You’ll work collaboratively with consumers, families and carers, GPs, residential aged care facilities and other health and community services. Our teams are committed to person-centred, recovery-oriented care, supporting older people to remain well and independent within their own homes and communities wherever possible. This means your work will extend beyond the office, with regular opportunities to undertake home visits, residential aged care facility visits and community-based assessments, while remaining connected to a supportive multidisciplinary team.
What You Will Be Doing
As a Registered Nurse within the Community Older Persons Mental Health Service, you’ll provide specialist nursing care across assessment, treatment, care coordination and ongoing clinical management.
You will:
- Undertake comprehensive mental health assessments, including mental state examination, psychosocial assessment and risk assessment.
- Provide care coordination for consumers with complex mental health and psychosocial needs.
- Develop, implement and review collaborative care plans with consumers, families and carers.
- Monitor mental state, treatment response, medication adherence, physical health and changes in risk.
- Provide nursing interventions, psychoeducation, medication support and other evidence-based interventions within your scope of practice.
- Undertake home visits, residential aged care facility visits and assessments across community settings.
- Work closely with psychiatrists, medical officers, GPs, families, carers, aged care providers and other health and community services.
- Participate in multidisciplinary clinical reviews, case discussions and treatment planning.
- Support consumers experiencing deterioration in their mental health and contribute to timely assessment and escalation.
- Contribute to clinical governance, quality improvement and service development activities.
- Build therapeutic relationships that support recovery, engagement and positive outcomes for older people.

About You
We’re looking for a compassionate, reliable and clinically curious Registered Nurse who enjoys working with older people and is interested in supporting consumers with complex mental health, physical health, cognitive and psychosocial needs.

Westmead Hospital is a key facility of one of Australia’s largest major health and medical research precincts and is the principal referral hospital for Western Sydney Local Health District providing district level health services to Cumberland, Parramatta and The Hills Shire LGAs including tertiary and quaternary services to greater Western Sydney, greater metropolitan Sydney, rural NSW and other states. The Westmead Health Precinct currently under development is envisaged to be the largest hospital building project undertaken in Western Sydney and will transform the iconic but ageing Westmead Hospital into an innovative, contemporary, and integrated centre that will continue to deliver high quality healthcare and expand the education, health, and research capability of the area.
Westmead Hospital is a leading centre of research, training and specialisation in all fields of Medicine and Dentistry, providing services in Cardiology, Cardiothoracic Surgery, Neurology, Neurosurgery, Kidney and Pancreas transplantation, Cancer Care, Obstetrics and Gynaecology, Neonatal Intensive Care, Trauma, Bone Marrow Transplantation, Critical Care, Aged Care and Rehabilitation, Adult Acute Psychiatry (networked with the co-located Cumberland Hospital) as well as other specialised medical and surgical services.
Westmead Hospital is also the principal referral centre for other metropolitan hospitals within the district including Auburn Hospital, Blacktown and Mount Druitt Hospital.
The Geriatric Medicine Department at Westmead Hospital provides a range of emergency, inpatient and outpatient services for patients who are 70 years or above presenting with wide range of clinical conditions usually on a background of multiple medical comorbidities and psychosocial issues. Comprehensive geriatric assessments are conducted by multidisciplinary teams in acute, subacute, community and residential aged care settings. HOPE which stands for Healthcare for Older Persons Earlier is a geriatric admissions and short-stay unit that provides acute assessment of elderly patients presenting to emergency. There are also dedicated services for stroke, falls and syncope, cognitive disorders, rehabilitation, orthogeriatrics, elderly trauma, perioperative care and aged care psychiatry. The department provides 24/7 assessment, intravenous thrombolysis and intra-arterial clot retrieval treatment to acute stroke presentations. Stroke patients are provided multidisciplinary care within the specialized acute stroke unit.
What you'll be doing
WSLHD - Advanced Trainee, Geriatrics - Westmead Hospital
- Trainees will be responsible for day to day patient care and for maintaining close liaison with the attending consultant medical officer. Patients will be managed in the various inpatient, outpatient and community- based programmes that offer interdisciplinary care. Patient assessments will be conducted face-to-face as well as via telehealth and documented using electronic medical records. Trainees will work collaboratively with allied health clinicians. Trainees will participate in multidisciplinary case conferences, family meetings and NCAT guardianship hearings.
- This position offers exposure to acute stroke medicine. There will be opportunity to work in the acute stroke unit, in stroke rehabilitation and attend weekly stroke education meetings. Trainees will participate in the afterhours stroke oncall roster. This role involves assessment of patients presenting with suspected stroke, review of cerebral imaging studies, administration of intravenous thrombolysis and coordination of intra-arterial clot retrieval procedures provided by interventional neuroradiologists. Supervision and formal training will be provided by consultant geriatricians and neurologists.
- Trainees also have the opportunity to undertake a term in psychogeriatrics in both inpatient and outpatient settings under the supervision of psychogeriatricians.
- Trainees will participate in teaching programs and quality improvement activities of the Geriatrics Medicine Department and the Division of Medicine. They will also provide education and supervision to medical students and junior medical officers.
- Trainees afterhours responsibilities include stroke oncall, ward medical registrar duty, weekend geriatrics wardrounds and evening ED admitting geriatric registrar (HOPE-ED) shifts.
- MBBS or equivalent, currently registered with the Medical Board of Australia.
- Sufficient clinical experience in order to undertake the clinical duties of the role in a variety of clinical settings and proficiency in basic and advanced life support. This includes completion of at least four postgraduate years and successful completion of the examination requirements of the Royal Australasian College of Physicians (RACP) and be eligible to undertake subspecialty training in Geriatric Medicine in Australia. Candidates who have completed FRACP part 1 (Written exam) and working towards completion of CRACP part 2 (clinical exam) will be considered for unaccredited advanced training
- Advanced ability to work effectively as part of a multi-disciplinary team in the planning, delivery and coordination of patient care.
- Advanced communication skills, including the ability to convey, and adapt verbal and written information to a clinical and non-clinical audience

Canterbury Hospital is seeking an Unaccredited Trainee in Cardiology to join our medical team for the 2027 clinical year. This opportunity will support training doctors to meet the needs of the Canterbury patient population whilst developing expertise in a complex clinical environment.
Where you'll be working
Canterbury hospital is a 215-bed metropolitan acute general hospital in the Sydney Local Health District providing services in Emergency Medicine, General Surgery, General Medicine, Obstetrics and Gynaecology, Paediatrics, Aged Care, Rehabilitation and Palliative Care. The facility is also undergoing a $350M redevelopment for expansion of acute clinical services planned in 2027. The Canterbury area is home to a culturally and linguistically diverse community catering to patients with complex health problems and limited access to health care. Canterbury Hospital therefore offers a unique opportunity for doctors in training to develop hands-on clinical experience whilst learning in a supervised environment.
What you'll be doing
The Unaccredited Trainee in Cardiology will provide effective clinical management of inpatients and outpatients of Canterbury Hospital, working under the supervision of consultant cardiologists to ensure quality health care for the local community, while developing expertise in Cardiology in a complex clinical environment.
The trainee will assist to manage common acute and chronic cardiac issues such as NSTEACS, acute arrhythmias and heart failure. The trainee will also contribute to performing, interpreting and reporting non-invasive cardiac investigations such as transthoracic echocardiography, stress echocardiography (and stress ECGs) and Holter monitoring. The trainee will play a key role in the Heart Failure Clinic. They will be involved in the assessment and ongoing management of a diverse range of heart failure patients, including those with newly diagnosed cardiomyopathies, myocarditis, and cardiac amyloidosis. There will be opportunity to contribute to clinical research projects through the rotation, along with opportunities for exposure to cardiac genetic testing, cardiac MRI interpretation, and the management of complex cardiomyopathies under supervision.
The role includes patient assessment, making treatment decisions, performing procedures, maintaining adequate medical records, organising and interpreting appropriate investigations. They will work under the supervision of Cardiologists and will be expected to have advanced communication skills, including escalating matters of concern in a timely manner, according to the clinical urgency.

- MBBS or equivalent, currently registered or eligible for registration with the Medical Board of Australia.

Westmead Hospital is a key facility of one of Australia’s largest major health and medical research precincts and is the principal referral hospital for Western Sydney Local Health District providing district level health services to Parramatta, Holroyd and The Hills Shire LGAs including tertiary and quaternary services to greater Western Sydney, greater metropolitan Sydney, rural NSW and other states. It is a principal referral centre for other metropolitan hospitals within the district including Auburn Hospital, Blacktown and Mount Druitt Hospital.
Westmead Hospital is a leading centre of research, training and specialisation in all fields of Medicine and Dentistry, providing services in Cardiology, Cardiothoracic Surgery, Neurology, Neurosurgery, Kidney and Pancreas transplantation, Cancer Care, Obstetrics and Gynaecology, Neonatal Intensive Care, Trauma, Bone Marrow Transplantation, Critical Care, Aged Care and Rehabilitation, Adult Acute Psychiatry (networked with the co-located Cumberland Hospital) as well as other specialised medical and surgical services.
The Westmead Hospital Emergency Department (ED) operates 24 hours, 7 days per week and manages all types and levels of illnesses and injuries. As one of six major trauma centres in the state, the ED also cares for people with the most severe physical injuries. Westmead Hospital ED presentation numbers are among the highest in the state, currently around 80,000 per year.
Westmead Hospital is one of Australia’s largest centres for postgraduate training to specialist levels in all fields with over 100 PhD students and other postgraduate students supervised by Westmead Hospital scientists and clinicians. Westmead is home to internationally recognised institutes of health research, the most prominent being the Westmead Millennium Institute. The Westmead Millennium Institute is one of the largest medical research institutes in Australia, conducting research into a wide range of human disorders affecting both adults and children, including infectious and immune diseases, cancer and leukaemia, liver and metabolic diseases, eye and brain related disorders and heart and respiratory disorders.
Cumberland Hospital, co- located with Westmead Hospital provides primary and secondary mental health services to the local population and tertiary acute, extended and forensic mental health services to greater western Sydney and NSW. There is also a range of community mental health residential facilities, recovery support and specialist treatment services located on the campus.
The Trainee works as part of a multi-disciplinary team which includes medical, nursing and allied health professionals. They will work at the Senior resident medical officer (SRMO) level or Junior Registrar level (Depending on level of experience).
The Trainee is directly responsible to the supervising Emergency Medicine Specialist of the day, with overall supervision by the Term Supervisor and the Emergency Department Director.
The Trainee must work effectively with other specialty teams and nursing staff in the wider hospital, to ensure optimal patient care. The Trainee must also interact professionally with clinicians outside of the hospital, including GP’s and community nurses.
What you'll be doing
WSLHD - Trainee Stage 1 - Emergency Medicine (Westmead)
The primary purpose of the Trainee in the Emergency Department is to provide clinical care, under appropriate supervision, to patients requiring emergency medical services in a complex clinical environment. This position will provide experience in a critical care environment and have responsibilities and opportunities for autonomous practice commensurate with the level of experience of the incumbent.
The Trainee is expected to function at the minimum of a Senior Resident Medical Officer (SRMO) level.
The following rotations are available to Trainees:
· Westmead Emergency
· The Children’s Hospital Westmead Emergency
· Westmead and Auburn anaesthetics
· Westmead ICU
· Other rotations may be negotiated depending on availability
It is expected that provisional trainees when eligible will apply to the Australasian College of Emergency Medicine (ACEM) for enrolment into training.
Westmead ED is accredited for provisional & advance training with 16 hours/7 days emergency physician cover.
Our teaching program has multiple streams dedicated to different requirements including a weekly registrar teaching. There are also skills days throughout the year specific for provisional trainees and preparation in both the primary and fellowship exam.
- MBBS or equivalent, currently registered or eligible for registration with the Medical Board of Australia
- Must have completed at least two postgraduate years training in an Australian Public Hospital or equivalent (not as an Observer)
- Demonstrated ability to work independently in a supervised environment with commitment to self-education and continuous professional development
- Ability to work effectively as part of a multi-disciplinary team with an understanding of the roles of the doctor and other members of the healthcare team.
- Ability and willingness to work at the facilities within the Network to deliver patient care. Ability and willingness to participate in a rotating roster including days, evenings, nights and weekends and preparedness to participate in an on call roster for the emergency department.
- Ability to recognise the acutely unwell patient and to initiate immediate management and institute Basic Life Support. Completion of ALS training, or willingness to complete in the first 3 months of appointment
- Excellent organisational and time management skills and ability to manage competing priorities in a complex environment.
- Demonstrated commitment to quality improvement, patient safety and risk management

- Assist services with audits, accreditation readiness and compliance with quality standards, regulatory requirements and the Pinangba Cultural Governance Framework.

- Bachelor of Nursing (or equivalent) with current unrestricted AHPRA registration and unlimited work entitlements - Essential.

About the Role
St Vincent's offers a unique and highly specialized paid Mental Health Cadetship Program encompassing adult and aged mental health, in acute and community settings. Mental Health Nursing Cadetships offer paid employment experience, professional support to students undertaking a Bachelor/Masters of Nursing degree. Cadets undertake shifts at St Vincent’s Hospital for a total of 30 shifts over the year to enhance the development of their skills and knowledge in mental health nursing, and assist with the successful completion of their studies. Cadets will work under the supervision of a Registered Nurse or Mental Health Clinician to observe and provide support across an array of clinical areas, within a defined scope of practice.
Duties include (but are not limited to):
• Supporting consumers throughout their recovery journeys and working alongside the multidisciplinary team
• Actively upholding a culture of hope by using optimistic language, supporting people and celebrating consumer’s recovery efforts;
• Supporting consumers with available resources to achieve their recovery goals through education and support around coping strategies and symptom management
• Actively challenging stigmatizing attitudes, discrimination and social exclusion in line with the Recovery Model of Mental Health Care.
• Attendance of rostered shifts during the employment period
• Ensures that all care delivered is at all times supervised by a Registered Nurse
• Communicates effectively with supervising nurses about individual skills and learning needs

About the role:
We’ve currently looking for a motivated Administration Officer (Coding) to join our busy team at St Vincent’s Private Hospital Toowoomba on a permanent part-time basis (27 hours per week).
