About the Role
Join CALHN as a Social Worker in the Transition Care Program and help older people achieve meaningful care outcomes. You’ll manage a complex caseload, deliver tailored therapy and work with a multidisciplinary team to support restorative, person-centred care.
- Assess needs and deliver tailored social work therapy
- Manage a complex caseload with sound clinical judgement
- Plan holistic care with clients, carers and service partners
- Build strong links across health, community and aged care
- Support service quality, research and team development
About You
Bring your social work expertise, sound clinical judgement and collaborative approach to supporting older people with complex needs. To be successful in this role you will:
- Hold a degree eligible for full AASW membership
- Hold or be eligible for a South Australian driver’s licence
- Bring experience managing complex older person caseloads
- Deliver social work assessment, therapy and intervention
- Work autonomously and within an interdisciplinary team

About The Role
Mount Gambier and Districts Health Service is seeking a Medical Practitioner (Adult Internal Medicine Registrar) to join our General Medicine team. Working closely with Consultants, Visiting Specialists, Nursing and Allied Health professionals, you will provide high-quality inpatient and outpatient medical care while contributing to safe, evidence-based clinical practice. This role offers valuable exposure to a broad range of medical presentations within a regional health service and includes opportunities to participate in teaching, supervision, quality improvement and ongoing professional development. You will play an important role in supporting patient outcomes while developing your own clinical expertise in Adult Internal Medicine.
About You
You are a motivated and patient-focused medical practitioner who enjoys working collaboratively within a multidisciplinary team. You bring sound clinical judgement, excellent communication skills and a commitment to delivering high-quality care while continuing to develop your professional knowledge and experience.
To succeed in the Medical Practitioner (Adult Internal Medicine Registrar) role, you will:
• Hold a Bachelor of Medicine (or equivalent), be enrolled with RACGP or ACCRM and have current AHPRA General Registration
• Demonstrate appropriate post-graduate medical experience
• Possess knowledge of contemporary medical practice and procedures
• Communicate effectively with patients, families and multidisciplinary teams
• Work effectively under pressure while maintaining high standards of patient care

The General Medicine Department provides comprehensive medical care to adult patients across inpatient and outpatient settings. Working closely with a multidisciplinary team, the department delivers high-quality, patient-centred care while supporting clinical education, training, research and continuous quality improvement activities. Conditions of Employment

This role delivers hospital-level care where it matters- in the Home and Communities of Adelaide’s Southern Suburbs. Flinders Medical Centre is seeking committed Clinical Nurses to join Hospital In The Home (HITH) program within the Southern Adelaide Local Health Network (SALHN), bringing expert clinical care directly to patients, who can have treatment safely outside of a hospital ward.
About the role:
As a clinical nurse within the HITH team, you will manage and independent patient case load across the Southern Adelaide Metropolitan area, coordinate and delivering acute-level care in the community and home settings, reporting to the Nurse Unit Manager of the HITH service.
Your key responsibilities will include:
- Providing evidence-based nursing care including experience with managing PICC, IVC and ports, IVC cannulation, administration of medications.
- Acute wound management skills.
- Completing comprehensive clinical assessment and escalating care in line with SALHN protocols.
- Collaborating with the multidisciplinary team to plan, review and transition patient care.
- Supporting patients and families with education to promote safe self-management and recovery.

About you:
- Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia AHPRA.
- Demonstrated experience in acute medical, surgical or community nursing with at least 3 years post graduate experience.
- Confidence working autonomously in unstructured and variable care environment.
- Sound clinical decision-making and prioritisation skill across a mixed patient caseload.
- Current unrestricted South Australian Driver Licence.
- Familiarity with SA Health clinical standards and electronic medical record systems.
- At least 3 years post registration.
In this role you will have strong interpersonal and communication skill which will enable you to successfully problem solve, resolve conflicts and negotiate, including the ability to engage in difficult conversations, whilst building and maintaining relationships. About the Role
An exciting opportunity exists for an experienced Clinical Pharmacist to join the Hospital in the Home (HITH) and Residential Inreach (RIR) services at St Vincent’s Hospital Melbourne. Working alongside the Senior Hospital in the Home Pharmacist, you will provide high-quality clinical pharmacy services to patients receiving acute care within their own homes and residential care facilities. This varied role offers the opportunity to work across hospital and community settings, supporting the safe, effective and appropriate use of medicines while working closely with patients, carers and multidisciplinary healthcare teams. As part of the role, you will travel to patients’ homes and residential facilities to provide clinical pharmacy services and support continuity of care outside the traditional hospital environment.
Your responsibilities will include:
- Providing clinical pharmacy services to HITH and Residential Inreach patients in their homes and residential care facilities
- Obtaining comprehensive and accurate medication histories and undertaking medication reconciliation
- Reviewing medication therapy and providing recommendations to support safe and optimal medication use
- Providing medication counselling and education to patients, families and carers
- Promoting the safe, timely and cost-effective use of medicines
- Collaborating closely with medical, nursing, pharmacy and other healthcare professionals
- Liaising with community pharmacies, general practitioners and other external healthcare providers as required
- Supporting safe transitions of care between hospital, home and residential care settings
- Maintaining accurate and timely clinical documentation
Salary: Classifications for this position will range from SX2–SX5 ($94,478.80–$106,906.80 per annum, pro rata for part time).
Your Contribution
You are an experienced hospital Pharmacist with strong clinical judgement, excellent communication skills and the confidence to work both independently and collaboratively within a multidisciplinary environment.
You will bring:
- A recognised Pharmacy qualification and current registration as a Pharmacist with AHPRA
- A minimum of three years' hospital pharmacy experience

We are seeking a dynamic and motivated Clinical Nurse (Education) with a passion for clinical teaching and facilitation. Our ideal candidate will have experience in Education and Cardiac Catheter Laboratory (CCL) to join our Education Team at St Vincent’s Private Hospital Northside.
This role is a Level 2 Clinical Nurse permanent, part-time position of 40 hours (0.5 FTE) per fortnight.
Reporting to the Education Manager, the Clinical Nurse (Education) is a vital member of the multidisciplinary team, responsible for promoting clinical excellence and staff development. This role supports the department’s vision of delivering evidence-based care through continuous learning and professional growth by addressing both organisational and learners' needs.
You will be responsible for all aspects of clinical education and training within the Cardiac Catheter Laboratory environment supported by a highly skilled multidisciplinary team. Your responsibilities will further include providing excellent person-centred development of staff, assessment of clinical skills, assisting with clinical education events and support of other education activities within the hospital in close collaboration with other members of the clinical education team.

About the opportunity:

You will play a crucial part in maintaining the cleanliness and hygiene standards of our facility, ensuring the well-being and comfort of elderly residents.
- General responsibilities include routine cleaning tasks such as mopping, vacuuming, dusting, and sanitizing common areas, resident rooms, and restrooms.
- Ensure the Aged Care home is cleaned and maintained in accordance with prescribed routines and infection control guidelines.
What you’ll bring:
- You’re reliable, resilient and have a whole lot of heart. Most importantly, you are genuinely interested in supporting others and making a difference in your community.
- Previous experience in cleaning or housekeeping roles.

Your Opportunity
Reporting to the General Manager Transformation, the Head of Transformation - Enablement partners with executive and business leaders to maximise the impact of Anglicare's transformation portfolio. You will provide leadership across transformation governance, portfolio management, risk and assurance, benefits realisation and executive reporting, ensuring strategic initiatives deliver measurable outcomes and sustainable organisational change.
This highly visible leadership role will work across multiple business units, helping senior leaders make informed investment decisions, prioritise strategic initiatives and strengthen organisational transformation capability.
With integrity and influence, you will:
- Lead the development and ongoing maturity of Anglicare's enterprise transformation governance framework
- Drive portfolio transparency, performance reporting and strategic prioritisation across transformation initiatives
- Provide enterprise-wide visibility of transformation performance, helping leaders make informed investment and prioritisation decisions.
- Partner with executive leaders to maximise delivery outcomes, remove barriers and realise the full value of strategic initiatives.
- Partner with business and finance leaders to establish benefits management frameworks and track value realisation
- Deliver executive and board-level reporting, insights and recommendations that support strategic decision-making
- Build organisational capability in transformation delivery, governance and portfolio management through coaching and leadership
What You'll Bring
You are an experienced transformation leader with a strong background in portfolio management and enterprise-wide change. You combine strategic thinking with exceptional stakeholder engagement skills and have a proven ability to influence senior leaders and drive outcomes in complex environments.
You are also passionate about fostering cultures built on integrity, compassion, excellence and service.

- Relevant tertiary qualifications
- Significant experience leading complex enterprise transformation programs and portfolios
- Strong expertise in portfolio, program and project governance
- Experience developing governance frameworks, controls and assurance processes
- Proven experience working closely with Executive Leadership Teams and senior stakeholders
· Strong analytical, reporting and decision-support capabilities

- Professional certifications such as MSP, PRINCE2, PMP, Agile, Prosci or similar
- Experience leading enterprise PMO, transformation enablement or portfolio functions
- Strong understanding of transformation risk management and benefits realisation frameworks
