About the Role
An exciting opportunity exists for a Registered Nurse or Grade 2 Allied Health Professional or to join the Transition Care Program at St Vincent’s. The successful applicant will be responsible for providing high quality, evidence-based Case Management support to aged care clients in Transition Care, working as part of a supportive, multi-disciplinary team. The role is based at St George’s Hospital in Kew and works as part of a close-knit, multi-disciplinary team. The Case Manager - Transition Care Program will assume a key role in the care coordination and case management of clients within the program. This will include conducting comprehensive holistic health assessments, coordinating health and social care needs, monitoring achievement of client goals, facilitating intervention from other health care professionals and ensuring appropriate linkages with hospital and community services. The Case Manager may also be involved in establishing ongoing community support services and identification of residential care options. This is a great opportunity for an enthusiastic Allied Health Professional or Nurse with aged care experience to join a cohesive and progressive integrated team, committed to excellence in clinical practice, research and professional development. The incumbent must hold, or be currently eligible for, membership with the relevant professional body. Relevant postgraduate qualifications will be highly regarded.
Classifications for this position will be: Social Worker Grade 2, SC21-SC24; Occupational Therapist VF6-VF9; Physiotherapist VB1-VB4; Speech Pathologist VW1-VW4 (90,000 - 104,000 per annum, pro rata for part time).
Duties include
- Provision of high quality, evidence-based, person-centred care, including the application of case management principles and practice frameworks.
- Collaborative care planning with clients, carers and/or families in the context of their health, functional, psychosocial, cultural, environmental and economic needs.
- Establish sound, collaborative working relationships and communicate effectively and professionally with a wide range of team members, TCP colleagues, key stakeholders and external service providers; to achieve complex discharge planning
- Support quality and continuous improvement initiatives, team processes and service development activities
Your Contribution
- Recent experience working in an Aged Care or subacute setting
- Membership of, or eligibility for, relevant professional body
- Demonstrated ability to communicate clearly and work effectively in a multidisciplinary team
- Proven ability to work collaboratively with a range of stakeholders, including clients, families, external agencies and other health professionals
- Highly developed interpersonal skills that incorporate the qualities of excellent communication, negotiation, organisation, planning and problem solving
- Knowledge of and commitment to continuous quality improvement processes
- Superior written and verbal communication skills
- Demonstrated knowledge of the application of case management practice frameworks and principles, in the context of the current health care environment.
- An ability to supervise students and/or other staff members as required
- Evidence of commitment to professional development in relevant areas

Transition Care Program
The Transition Care Program (TCP) is jointly funded by the Commonwealth and State and Territories. St Vincent’s TCP is resourced to follow the client through the service system and avoid unnecessary extension of hospital admissions and/or premature admission to residential care.
The intent of the Transition Care Program is to provide short-term support and active management for older people at the interface of the acute/subacute and residential aged care sectors.
The target group for Transition Care services at St Vincent’s are:
- Older people exiting from hospital who, via participation in the program, achieve further improvements to their physical, cognitive and psychosocial functioning and an improved capacity for living independently; and/or
- Older people exiting from hospital for whom the focus will be on actively maintaining their functioning while assisting them and their family/carers make appropriate long term care arrangements.
The Transition Care Program is located at the St Georges Health Service in Kew, but services all of the St. Vincent’s Hospital Melbourne catchment.

What you’ll bring
You’re a strong leader with a passion for people and service excellence.
To thrive in this role, you’ll have:
- Relevant qualifications with previous experience as a Chef or Cook.
- A hands-on leadership with the ability to lead by example in the kitchen when needed.
- Strong leadership skills with proven experience managing and coaching large teams.
- Experience in preparing and cooking for large groups.
- Skills in menu coordination and tailoring meals to diverse dietary needs.
- Knowledge of OHS, food safety standards, and regulatory compliance.

The Regional Manager ensures that the Dementia Support Australia (DSA) service is tailored to the needs of individual clients and ensures the service delivers on its key performance indicators.

Responsibilities
- Provide professional, capable and aligned leadership for the DSA service utilising HammondCare’s Mission and Mission In Action
- Establish and maintain clinical supervision, governance and behaviour management systems as part of a national team, with a local and borderless service delivery ethos
- Ensure the service meets its administrative, financial, risk and quality and reporting goals of the DSA program through a team-based approach
- Significant stakeholder engagement during start up and ongoing with key stakeholders in State Government, the aged care sector and acute care sector leadership teams to provide details on the program, ensuring smooth partnerships for service delivery, problem solving as needed and ongoing engagement during program delivery.
- Supporting Team Leaders to manage stakeholder engagement at the service level through local management in aged care and hospital settings.
- Support the National Program Manager, Team Leaders and Clinical Lead to develop strategic plans for each State and be part of the solution to continuous improvement of service delivery.
- Manage and motivate the service delivery team and ensure they are service and missionally aligned
- Leadership on the utilisation of data management and reporting systems and ICT platforms to support and enhance service delivery
- Provide high levels of service quality through clinical supervision, governance and behaviour management role modelling
- Support the National Management structure to provide line management and clinical governance to DSA
What do we need from you?
- Commitment to engage and align with HammondCare’s Christian Mission, Motivation and Mission in Action
- Tertiary qualifications in Nursing / Allied Health or a Health-related degree
- Desirable evidence of study and or qualifications in dementia
- Eligibility for registration as a Health Practitioner in Australia (AHPRA)
- Demonstrated high level experience and knowledge in supporting older people and carers living with dementia, with particular emphasis on psycho-social approaches to case management
- Previous experience working at a senior level in a multi-disciplinary or multi-site environment including budgeting skills, systems thinking, IT systems experience
- Strong ability to manage, motivate and develop a team and effectively work with other staff
- Willingness to travel intra and interstate. Given this program is in start up phase, this could involve travel away every month.
- Strong negotiation and conflict resolution skills
- High level communication and interpersonal skills, particularly between industry and government

Your part in the team

As one of our Living Support Partners (Personal Care Assistant) you will be part of an experienced, engaged and dedicated team that is committed to creating a welcoming, professional and caring environment for the residents of St Vincent’s Care Corinda. Living Support Partners provide direct care and assistance to resident with all activities of daily living. They partner with residents to support their daily routine, lifestyle and choices and ensure they feel safe, valued, respected and engaged. This position is to work as an effective household member and is supervised, providing care for residents in a specific household. They are responsible for assisting and supporting their wellbeing through professional, caring and efficient execution of required duties.

Certificate III in Individual Support/Aged Care or similar qualifications essential
Availability to follow below roster:
Week 1 and 2: Mon-2230-0630, Wednesday -1600-2100, Thursday- 2230-0630, Sunday - 1600-2100
12 months+ experience in a similar role with residential aged care experience (dementia experience) highly regarded
Excellent customer service and communication skills
Experience with AutumnCare (or similar software) with ability to maintain excellent records
Understanding of Aged Care, resident rights/responsibilities with ability to follow the care plan
Nationally Coordinated Criminal History Check (NCCHC)
NDIS Worker Screening Check may be a requirement of this role.

Including a multidisciplinary team headed by an enthusiastic Geriatrician, nursing staff, physiotherapy, occupational therapy, speech pathology, social work and pastoral care worker.
About the Role:
- Mercy Health Albury – Geriatric Evaluation and Management Inpatient Unit.

- To provide best practice care as a pivotal member of the multidisciplinary team.
- Act in a position of advocacy for the patient and their family.
- Be a part of this compassionate team in providing quality care.
About You:
- Essential requirements: Current registration with the Australian Health Practitioners Regulation Agency (AHPRA) as a Registered nurse.
- Must have current immunisation of diseases as listed in NSW Health Policy Directive. This is a Category A Risk position.
- Experience in Rehabilitation desirable but not essential.
- Focused on patient centred nursing.
- Minimum 2 years post graduate nursing experience.
- Demonstrate time management and organisational skills.
- Computer literacy.
You will also be required to provide evidence of, or in the process of obtaining:
- A current Police Record Check.
- Proof of current influenza vaccination and other vaccination status consistent with public health orders.
