About You
The registered nurse provides evidence-based nursing care to people of all ages and cultural groups, including individuals, families and communities. The registered nurse assesses, plans, implements and evaluates nursing care in collaboration with individuals and the multidisciplinary health care team so as to achieve goals and health outcomes. The registered nurse takes a leadership role in the coordination of nursing and health care within and across different care contexts to facilitate optimal health outcomes. This includes appropriate referral to, and consultation with, other relevant health professionals, service providers, and community and support services. The registered nurse practices independently and interdependently, assuming accountability and responsibility for their own actions and delegation of care to enrolled nurses and health care workers. Delegation takes into consideration the education and training of enrolled nurses and health care workers and the context of care.

The Role
Are you an experienced mental health nurse looking for a rewarding opportunity to make a meaningful impact in the lives of older people?
We are seeking a compassionate and motivated Registered Nurse to join our Older Persons Community Mental Health Team. As part of a supportive multidisciplinary team, you will deliver recovery-oriented, person-centred care to consumers aged 65 years and over who are experiencing mental illness, supporting their wellbeing, independence and quality of life within the community.
This varied and fulfilling role involves working collaboratively with consumers, families, carers, aged care providers, general practitioners and other healthcare professionals to provide comprehensive mental health assessments, care coordination and evidence-based therapeutic interventions across a range of community settings.
You will be part of a dedicated team of nursing and allied health professionals committed to achieving positive recovery outcomes, promoting mental wellbeing and enhancing the lives of older people in our community.

- Computer literacy, along with an understanding of the EMR recording system.

About the Role
Monash Health Aged Care Services is seeking a passionate and experienced Case Manager to join our Community Support Options team. Based within our Home Care Package (HCP) program, you will play a key role in supporting older people and their carers across the Casey, Cardinia, Greater Dandenong, Kingston, and Bayside municipalities. About the opportunity:
At Mercy Health, each moment, no matter how small, truly matters - and as a Registered Nurse G3, you’ll create cherished moments each day.
You will be an experienced Registered Nurse supporting our clients to live independently at home by providing complex nursing care to meet individual needs, to develop and implement nursing care plans and to assess, review and evaluate consumer care on a regular basis.
You will provide clinical oversight and assessments for our more complex clients, and for those clients without a case manager.
You will provide support to the nursing team regarding DVA funding streams
You will also provide support in the orientation of new staff.
What you’ll bring:

To thrive in this role, you will have:
Previous experience in the home care sector
Excellent clinical, time management, organisational and communication skills, with the ability to adapt and problem solve.
Excellent complex client problem solving skills.
A good understanding of community resources available to clients.
A compassionate, flexible, and positive approach to effective client care and management.
The ability to nurture friendly, respectful relationships and contribute to a collaborative team environment.
You will also be required to provide evidence of, or in the process of obtaining:
Evidence of Nursing Registration (AHPRA).
Current Victorian Drivers Licence.
NDIS screening check.
