The position focuses on program liaison functions related to patient discharge planning referrals, assessment of eligibility for community services, facilitation of referrals, generation of clinical care plans for community care, general enquiries and communication with referrers and the external contracted service providers.
- The Metropolitan Referral Unit Liaison manages the day-to-day referrals and contacts with stakeholders under the supervision of the MRU Nurse Unit Manager RN3 and Nursing Director- Program Director RN5. The Metropolitan Referral Unit Liaison (RN2 Clinical Nurse) is accountable for own practice standards, activities delegated to others and the guidance and development of less experienced staff.
- Provide nursing and/or midwifery services in a variety of health service settings which has been consolidated by experience and/or further study with staff at this level developing from competent to proficient practitioners.
- Accepts accountability for their own practice standards, activities delegated to others and the guidance and development of less experienced staff.
- The Clinical Nurse role at this level continues to be predominantly clinical in nature; however, employees are assigned appropriate portfolios. The allocation of portfolio responsibilities should be negotiated with each employee and be consistent with the career development plan for the employee as determined by their performance review/development plan.

As a Leisure and Lifestyle Officer you will be responsible for the assessment, planning, implementation and evaluation of individualised leisure and recreational programs for our residents in conjunction with the Lifestyle Coordinator. These programs will consist of a range of one-to-one programs, small group and large group activities including events and specialised activities all driven by the needs, wants and desires of the residents.
What you'll be doing- Undertake resident assessments and complete associated documentation.
- Design and facilitate the implementation of individualised leisure and lifestyle programs according to clients' social, emotional, cultural, spiritual, physical and cognitive needs and abilities. This includes the provision of one-to-one activities, group activities and special events.
- Assist residents to achieve maximum independence, maintain friendships and participate in the community through planned group and individual leisure-based activities.
- Monitor and evaluate the program on a regular basis to ensure that it is meeting the needs of the residents, including suggestions on any improvements.
- Facilitate the outing/bus trip program and complete associated documentation.
- Actively seek feedback and suggestions from residents and use this to guide quality improvement.
- Support the Leisure and Lifestyle Department to meet accreditation requirements.
- Work as part of a team to deliver high quality engagement and purposeful living programs and opportunities.
