Job Description:
About the opportunity:
St Vincent's has been a leader in Australia's health and aged care landscape for more than 165 years. Since our founders, the Sisters of Charity, opened our first hospital in 1857, our services and nearly 30,000 people have been behind some of Australia's most important medical breakthroughs. We are a microcosm of Australia’s health and aged care system and are uniquely positioned to lead and respond to our rapidly changing environment.
You'll be joining St Vincent’s as we launch our exciting new vision Bringing Care Home, which will take the best of St Vincent’s care beyond our hospitals and aged care homes directly into the homes of our communities across the country, making care more personal and accessible. Bringing Care Home is more than a strategy. It’s how we’ll deliver care that is better, fairer, and more connected – for everyone we serve.
Reporting to the General Manager, Ageing Care at Home, the National Manager – Home Aged Care will lead the national delivery of St Vincent’s community aged care services, including Support at Home, the Commonwealth Home Support Programme (CHSP), fee-for-service and future community-based services.
This is a key operational leadership role responsible for ensuring the end-to-end business operates effectively. You will lead operational, workforce, client and financial performance while ensuring services remain safe, person-centred, sustainable and aligned with the Aged Care Act, Aged Care Rules and strengthened Aged Care Quality Standards.
Working across St Vincent’s, you will also lead continuous improvement and technology-enabled change, strengthen operational governance and controls, and translate our Bringing Care Home strategy into high-quality, sustainable services for older Australians.
This is a permanent, full-time position, based in Sydney, Melbourne or Brisbane.
We are looking for a special kind of person, a St Vincent’s kind of person. Someone who puts excellence, compassion, connection and caring for other people at the heart of everything they do.
Key responsibilities of the role:
- Lead the national end-to-end operations of Home Aged Care, ensuring people, processes, systems and associated providers work together effectively.
- Drive strong operational and financial performance, including budget, utilisation and productivity, and ensure operational processes support accurate and timely claiming and client statements.
- Lead high-quality delivery of Support at Home and CHSP, ensuring services meet the Aged Care Act, Aged Care Rules, strengthened Aged Care Quality Standards and relevant program requirements.
- Build and lead a high-performing national workforce, developing capability, accountability and a strong culture through ongoing change.
- Partner with Quality, Clinical Governance, Compliance and Assurance teams to embed effective governance and controls and lead the business through audit, accreditation and assurance activity.
- Oversee associated provider arrangements and performance, ensuring services meet St Vincent’s regulatory, quality and contractual requirements.

Clinical Lead - Complex Care (521364)
Department of Health
Hospitals & Primary Care - Hospitals Nth
Hospitals North - Allied Health North
Applications must be submitted by Sunday 15 November, 2026 11:55 PM
Position details
The Team
The Complex Care team is an integral, highly valued tea of multi-disciplinary allied health professionals, applying a collaborative transdisciplinary approach to achieve innovative solutions to complex issues surrounding patients.
Everyone has a different definition of complexity, but for our team, complexity is very much about exceptional circumstances that are above and beyond usual practice for the health care teams working directly with the patient. This means that multi-medical or psycho-social presentations are not routinely considered complex by Complex Care.
Often our patients will have no clear hospital care pathways or sustainable discharge options back into the community, and many systems and services are involved that can lead to fragmentation of care and poor patient experience. The CCC team works with the patient, their supports, and the healthcare team to identify and coordinate care pathways, support patient flow, resolve barriers to discharge, avoid re-presentation to hospitals, reduce the burden of system pathway-finding on health care teams, and promote partnerships with patients and successful working relationships with the healthcare team.
The Role
We are seeking an experienced allied health professional to join our team based predominately in the rehabilitation service at Launceston General Hospital, to work alongside nursing, medical and allied health teams, and in close collaboration with external service providers, to support the right place right care patient journey and quality outcomes for our patients.
The successful applicant will provide expert clinical advice on care management and systems pathways, complex discharge planning and hospital avoidance for patients with exceptional needs.
You will need
- To be an experienced clinician who can confidently lead complex patient journeys, support high-risk discharge planning, and influence outcomes across multiple services, agencies, and stakeholders to achieve safe and sustainable patient pathways.
- Candidates should be prepared to provide clinical leadership beyond individual case management, including coaching multidisciplinary teams, driving service improvement initiatives, analysing outcomes and data, and contributing to system-wide solutions for patients with highly complex health, psychosocial, behavioural, and NDIS-related needs.
- Advanced clinical reasoning skills, sound judgement in ambiguous and evolving situations, strong knowledge of complex service systems, and the ability to build collaborative relationships, solve problems creatively, and drive timely patient-centred outcomes within a dynamic healthcare environment.
Note: - Please refer to the Statement of Duties for all the selection criteria of this position to address in your application.
