Monash Health exists to reimagine care, so that people can live their best lives.
We provide safe and equitable care, for every stage of life, delivered with world-leading expertise and over 175 years of experience. Our team of 24,000 people are full hearted, open minded and all in. Together, we provide more than 250 integrated services for every stage of life at over 50 locations and in the community across south-east Melbourne, Victoria, serving 1.2 million residents in the South Metro Local Health Service Network.
To see first-hand what our colleagues think about working here, take a look at the following short videos monashhealth.org/careers.
About the ED Mental Health & AOD Response at Monash Health (EMAAP- Emergency Mental Health, AOD, Allied, Peer Team)
The MMC Emergency Department has an inter professional team which brings together clinicians from mental health, alcohol and other drug (AOD), Social Work and Lived Experience to provide specialised and integrated care to patients presenting to the ED with high acuity mental health and AOD issues, and complex psychosocial needs. This service is called the Mental Health and AOD Response or EMAAP and is located within the emergency department. The team will provide a seven day a week, extended hours service as well as outreach community follow up and support for a 28-day period post patient discharge.
About The Role
Grade 3 Social Worker | 0.6 EFT | Monash Medical Centre | Ongoing, Part-Time
Ready to take your social work practice into a role where clinical expertise, creativity and genuine human connection can change the course of someone’s day—and sometimes their life?
Join the EMAAP Team at Monash Medical Centre, supporting people presenting to the Clayton Emergency Department with behavioural health concerns, complex psychosocial needs and situations that demand more than a ‘quick fix’. This is social work at its most dynamic: clinical, collaborative, fast-paced and deeply person-centred. Be the clinician who helps make the next step possible.
As a Grade 3 Social Worker, you’ll bring your experience, judgement and therapeutic skills to provide high-quality psychosocial care across the ED journey—and beyond. You’ll have the opportunity to:
- Provide senior clinical care to people experiencing behavioural health and complex psychosocial challenges.
- Complete comprehensive psychosocial assessments, brief interventions and collaborative care planning to understand the whole person—not just the presenting crisis.
- Provide short-term assertive outreach for up to 28 days post-discharge, helping people stay connected when continuity of care matters most.
- Turn complex situations into practical pathways forward, reducing psychosocial stressors and supporting safe, timely discharge.
- Build strong connections with community services, health providers and support networks, creating care that extends well beyond the ED.
- Work with medical and nursing teams, EPS (Emergency Psychiatric Service) clinicians and peer workers, bringing the unique social work lens to complex clinical presentations.
- Lead, educate and supervise, sharing your expertise and helping strengthen social work and interdisciplinary practice across the service.
This is an opportunity for an experienced senior social worker who enjoys clinical complexity, autonomy and collaboration—someone who can think critically, build rapport quickly, navigate systems confidently and advocate fiercely for the person in front of them. You’ll be part of a team that values recovery, partnership, innovation and the power of a strong therapeutic relationship. And because this is a senior clinical role, you won’t just be delivering care—you’ll help shape it.
Why you’ll love this role
Every day is different. Every person brings a different story. And your expertise can make a tangible difference. If you’re looking for a role where you can combine clinical social work, crisis intervention, assertive outreach, leadership and genuine multidisciplinary collaboration, this could be your next move. Bring your experience! Bring your curiosity! Bring your social work voice! Join the EMAAP Team at Monash Medical Centre and help people move from crisis toward their next chapter.
About You
You are an experienced Grade 3 Social Worker with strong clinical expertise within acute or emergency settings. You bring advanced skills in psychosocial assessments and are confident working with individuals experiencing complex presentations and psychosocial stressors.
You thrive in fast-paced environments and build strong, collaborative relationships with medical, nursing, mental health and peer support colleagues. Your practice is recovery-oriented and person-centred, balancing choice and self-determination with medico-legal requirements and duty of care. You are skilled in short-term, assertive outreach and care coordination, facilitating timely discharge and meaningful community linkages. As a senior clinician, you provide supervision, contribute to clinical leadership, and actively support a collaborative, high-performing team culture.
Qualification & licence requirements are as per the attached supportive documentation.
Why Join Us?
At Monash Health, you'll have the opportunity to use your skills in a role that combines the pace and complexity of emergency care with the continuity and connection of community-based support. Here's what makes this opportunity special:
- Make an impact beyond the ED: Support people through their immediate crisis and help them stay connected to care after discharge.
- Embrace clinical variety: Work with diverse presentations and complex circumstances that will challenge and strengthen your clinical expertise.
- Bring your social work voice: Be part of an integrated team where your holistic perspective, advocacy and professional judgement are valued.
- Lead and inspire others: Share your knowledge, provide clinical guidance and contribute to the development of colleagues and the broader service.
- Be part of something meaningful: Join a passionate multidisciplinary team committed to compassionate, collaborative and person-centred care.
- Continue to grow: Access professional development, leadership opportunities and the benefits of working within one of Victoria's largest public health services.
You'll also enjoy access to Monash Health employee benefits, including salary packaging, wellbeing initiatives, professional development opportunities and a supportive workplace culture.

About the Role
Wesley Mission Queensland is seeking a Casual Registered Nurse to join our Supported Disability Accommodation (SDA) team.
This hands-on clinical role involves delivering high-quality, person-centred care within small, shared home environments where people with disabilities are supported to live independently and with dignity. Rather than working in a large residential or hospital setting, you will build meaningful relationships with participants living in their own homes, providing tailored clinical support that reflects their individual needs and goals.
As a Registered Nurse specialising in High Intensity NDIS supports, you will work with participants with complex needs, including enteral feeding, catheter and bowel care, tracheostomy and ventilation support, dysphagia management, and other specialised interventions.
You will also provide clinical oversight and guidance to ensure safe, high-quality care across the support team.
Key Responsibilities
- Provide direct nursing care to residents
- Conduct comprehensive clinical assessments
- Develop, implement, and review care plans
- Medication management and oversight
- Supervise and support care staff and ENs
- Ensure safe, compliant, and person-centred care
- Contribute to staff training and clinical guidance
- Maintain strong accountability across NDIS and professional nursing standards
- Manage high-acuity, complex clients with increasing clinical fragility
- Operate as the sole RN on site at times, with high autonomy and decision-making responsibility
What We Offer
- Salary packaging up to $18,550 (increase your take-home pay)
- Flexible shifts to support work-life balance
- Free, secure on-site parking
- Ongoing training and development
- Supportive and collaborative team culture
- Opportunity to work with a values-driven not-for-profit
Permanent RN Pay Rates
- Mon–Fri (AM shifts): $39.05 – $49.58 per hour
- Mon–Fri (Afternoon shifts): $43.93 – $55.78 per hour
- Mon–Fri (PM shifts): $44.91 – $57.02 per hour
- Saturday: $58.58 – $74.37 per hour
- Sunday: $78.10 – $99.16 per hour
- Public Holidays: $97.63 – $123.95 per hour
The Reality
This role is rewarding — Like any healthcare position it can also be challenging at times.
- Fast-paced, sometimes complex clinical environment
- Requires sound clinical judgement and resilience
- Working with diverse and evolving care needs
You’ll be supported by a strong team and leadership — and you’ll see the real impact of your work.
About You
- Registered Nurse with a passion for person-centred care
- Strong clinical assessment and communication skills, with the ability to work autonomously in a complex environment
- Calm, resilient, and confident in decision-making
- Collaborative team player
Essential Requirements
- Current AHPRA registration
- Demonstrated clinical nursing experience (disability desirable)
