Community Nurse – Virtual Care Team
Take your nursing career beyond the traditional clinical setting. Make a difference where it matters most — in the community.
Are you a skilled, compassionate nurse who is passionate about delivering person-centred care and helping patients achieve better health outcomes?
Join our forward-thinking Virtual Care Team as a Community Nurse and play a vital role in supporting patients as they transition from hospital to home, promoting independence, and helping prevent unnecessary hospital admissions.

With a focus on health promotion, early intervention, and continuity of care, your contribution will make a meaningful difference to the lives of those you support.
Why Join Us?
If you’re ready to embrace a modern approach to healthcare, work collaboratively across disciplines, and help people remain well and independent in their own communities, we’d love to hear from you.
Bring your expertise. Embrace innovation. Make a difference in community care.
