The Senior Case Manager will focus on the provision of intensive levels of support and case management to young people aged 12 -18 who are most "at risk" in the community. These young people are subject to statutory orders either with Child Protection or Youth Justice, and Berry Street provides the contracted case management in accordance with case planning and Children's Court directions.
You will be:
- Providing contracted case management of children and young people (generally clients on statutory protective orders) in accordance with Children's Court directions.

The Birthing in Our Communities (BiOC) program provides a coordinated and culturally safe support system for Aboriginal and Torres Strait Islander families during pregnancy and childbirth. By offering a comprehensive mix of services tailored to family and community needs, we aim to build family capacity, strengthen community connections, and safeguard the wellbeing of children. At ATSICHS, through holistic case management, and collaboration with multiple service providers, BiOC empowers families to navigate challenges, develop essential life skills, and access therapeutic support to improve maternal and infant health outcomes for Aboriginal and Torres Strait Islander Families. The program also engages with families involved in the child protection system, working to address safety concerns, facilitate reunification, and support positive outcomes for children and their families.
Duties
- Support families in crisis - by delivering targeting and brief interventions that build the individual client's ability to regulate and cope with stress and family pressures.
- Build strong relationships with clients through effective micro-counselling techniques (body language, voice tone, active listening) and delivering unconditional positive regard in removing shame or blame perceptions of client beliefs.
- Facilitate evidence-based and culturally appropriate health promotion and parenting education to BiOC mothers, families and FSW's - both individual and group settings as outlined by the manager.
- Deliver immediate safety assessment and safety responses, documented and approved by the Line Manager, in instances of domestic and family violence, self-harm/mental health, and child abuse.
- Facilitate ATSICHS 'Narrative Assessment Tools' completed in full by client ensuring the voices of clients are imbedded in their assessment. Facilitate conversations and introspection on all assessment domains.
- Ensure families are provided care throughout pregnancy, birth, and postnatal period, in collaboration with clients' assigned midwives and appropriate care service providers. Ensure client is supported through the 3 stages of BiOC schedule of care - Antenatal Care, Postpartum Care and Infant Development.
- Ensureall children meet developmental milestones by facilitating family access to ATSICHS paediatric/Mums and Bubs medical services, including access to specialist allied health supports.
- Enrolment in Kindergarten and family participation in early years playgroups.

In this role, you will provide developmentally appropriate therapy and conduct assessments for children, young people, and their families. Your expertise will help inform practical recommendations for clinical, educational, and family support. Regular assessment work will be a core part of your weekly practice, including cognitive, developmental, autism, ADHD, and adaptive functioning assessments. You will work closely with colleagues across disciplines to ensure a holistic approach to care.
Requirements- General registration with AHPRA and eligibility for a Medicare provider number
- Experience assessing children and young people, including report writing
- Proven ability to work collaboratively with families and across teams
- Knowledge of child safe practice, child protection, and mandatory reporting
- Current Working with Children Check and police checks

The BiOC Team Leader will support the Program Manager in the effective implementation of the Jajumbora BiOC Model of Care by managing and coordinating the Family Support Workers (FSWs) within the Hub. This hands on role involves direct engagement with staff and clients, ensuring that comprehensive and culturally informed maternal and child health services are provided.
The position is not only hands on but also deeply rewarding, offering the chance to make a meaningful and impactful difference in the lives of Aboriginal and Torres Strait Islander women and families.
Duties
- Establish and maintain regular reporting activities related to client record systems.
- Conduct regular audits of Family Support Workers' (FSWs) caseloads and facilitate high risk case conferences with FSWs.
- Lead incident debriefing processes.
- Provide monthly supervision sessions with FSWs.
- Support the team in working confidently within the BiOC Model of Care.
- Offer ongoing education and support to ensure compliance with organisational policies and procedures.
- Develop individual Professional Learning Plans for team members, including clear goals and actions.
- Lead targeted coaching to enhance team performance.
- Enhance communication within the team to promote ATSICHS vision and core values. Foster a positive team culture through targeted reinforcement and praise.
- Maintain high consistency and accountability for scheduled tasks in LogiQC. Ensure all scheduled tasks are completed within specified action day.
- Support and manage a continuous improvement model to enhance service delivery and staff practices.
- Oversee the daily implementation of the Jajumbora BiOC Model of Care, ensuring alignment with program guidelines and objectives. Adapt the model as needed to address specific community needs and feedback.

Desired Skills and Experience Required
- Bachelor's Degree or Diplomain Social Work, Human Services, Psychology, Sociology, Criminology, Counselling, or relevant social services field with at least 5 years' experience in Child Protection, Family Support, Youth Support or relevant human services field.
AND
- Demonstrated experience in overseeing and coordinating case management processes and procedures.
- Ability to lead, guide and manage people including the implementation of cultural change and change management opportunities.
- Ability to work in a performance driven environment and meet month-to-month program targets and performance outcomes.
- Ability to support and manage a continuous improvement model to further improve the service model and practice application of front line staff.
- Further qualifications or experience in settings related to maternal, child health, or family services.
- Experience in developing and implementing health or community service programs.
- Familiarity with local services and networks within Brisbane and Logan, including other health and social service providers.

- Current Working with Children's Card (Blue Card) or be eligible toapply.
- Driver's Licence.
- Evidence of Work Rights.
- Immunisations Category A.
