Responsibilities
The Dementia Support Coach will facilitate on the ground support, implementation of relationship-based support strategies, modelling and coaching with clients, carers and care staff.
Work collaboratively with peers to develop the skills, expertise and/or applied knowledge of aged care staff and carers to understand the person living with dementia and develop best practice support and care.
Support the implementation of the recommendation plans on the ground, drawing on modelling and coaching techniques, and seek guidance from Team Leader, Dementia Consultant and Senior Consultant as required.
Facilitate and support the trial and implementation of brokerage items with clients and their carers, and seek guidance from Team Leader, Dementia Consultant and Senior Consultant as required.
What do we need from you?
Strong organisational abilities, including effective time management and ability to prioritise in a busy work environment and competing demands.
The ability to work both independently and as part of a team.
Demonstrated commitment to excellence in customer service principles including conflict resolution to ensure the consistent provision of exceptional service to internal and external customers
A current driver’s licence and safe driving record
Willingness to travel and work intrastate and interstate to support the delivery of DC services
Qualification in relevant field; Allied Health assistant, Diversional Therapy, Enrolled Nursing (EN), Aged Care, Cert III or IV in Individual Support / Leisure and Health or similar highly desirable

About the role
The Clinical Partner (Home Care Registered Nurse) works within HammondCare’s Mission in Action to deliver high-quality, person-centred clinical care that supports clients to live well in their own home.
The role provides evidence-based, best practice clinical care holistically, to individuals, ensuring their unique goals, needs and preferences are understood and addressed. This role is focused on assessing clinical care needs, developing, and reviewing care plans and delivering excellent clinical care in partnership with clients and key stakeholders.
The role works collaboratively with the care management team to ensure the best outcomes for clients in the community setting and provides guidance to care staff to ensure the safe and continuous delivery of high-quality clinical care. The clinical partner will be travelling to Balliina to Tweeds Heads /Gold Coast but will be required to cover leave and attend to urgent referrals in Lismore, Kyogle and Casino if the other staff are on leave .

About you

Nursing qualification and current registration with AHPRA as a Registered Nurse (Division 1, General).
A current driver’s license and the ability to travel between HammondCare’s sites and services.

The Opportunity
HammondCare’s Respite Cottages provide short-term accommodation and individualised support for older guests during stays of up to 10 nights. As a Specialised Carer, you’ll play a vital role in delivering personalised care and creating a welcoming, home-like environment.
Please note: Rosters available may not suit student visa due to hour restrictions. Unlimited work rights will be highly regarded.
About the role
Hourly Rates: $32.91 to $36.50 base + Super + Casual Loading
Casual Shifts include:
7.15am-14.45 - Morning shift
14.45 - 21.45- Afternoon shift
21.30- 07.45- Night shift
*** Ideally you will be able to work a mix of the above shifts
Responsibilities
Provide personal care including grooming and hygiene
Deliver care in accordance with individual care plans
Support and care for guests during overnight or extended stays
Promote guest dignity and independence through daily activities
Maintain accurate documentation and communicate changes in care professionally
In some cases, you’ll have an opportunity to go offsite and provide care and supervision for our Clients, and this would mean having the capacity to drive a 12-Seater Bus (standard drivers license)
What do you need to apply?
Australian Drivers license
No formal qualification is necessary; however, Certificate III Individual Support is an advantage
Available to work a mix of the above mentioned shifts
First Aid certificate desirable
Empathy and compassion are a must
We provide job training as long as you have the willingness and desire to learn
Good administrative and computer skills, including the use of a smartphone

About The role

Certificate IV in Aged Care or other relevant qualification.
Experience in Community Aged Care management
