- Participate in care team meetings, case conferences, and continuous quality improvement activities.
- Promote WMQ services and build strong networks to support business growth and positive customer outcomes.
What We’re Looking For
You are a compassionate, organised, and customer‑focused professional who thrives in a fast‑paced, community‑based environment. You communicate clearly, build trust easily, and are passionate about supporting older people to live well at home.
Essential:
- 3–5 years’ experience in aged care, community services, or care/case management.
- Minimum Certificate III in Aged Care, Community Services, or a related field.
- Experience in customer engagement, service coordination, and working within regulated funding models (e.g. CHSP, aged care).
- Strong understanding of Consumer Directed Care, wellness, and reablement principles.
- Highly developed organisational and time‑management skills.
- Solid computer literacy, including Microsoft Office and client management systems.
- Current Driver’s Licence and willingness to travel as part of the role.
