Title: Clinical Nurse Consultant - Chronic Kidney Disease and Dialysis

Location: North Tasmania

Salary: $118,297 to $125,543 per annum (pro rata) + 12% superannuation contribution.

The Position

Join the Department of Health as a Clinical Nurse Consultant – Chronic Kidney Disease and Dialysis, providing expert clinical leadership and education within a multidisciplinary renal team. In this rewarding role, you will support people living with chronic kidney disease (CKD) and their families through education, care coordination, and service development, improving patient outcomes and continuity of care across the renal care journey. Working collaboratively with nephrologists, nursing staff, and allied health professionals, you will lead CKD patient education initiatives, coordinate dialysis access referral pathways, and contribute to quality improvement, research, and evidence-based practice within the renal service.

Key Responsibilities:

• Deliver a nurse-led chronic kidney disease education service.
• Provide education and support to patients and their families regarding CKD management and treatment options.
• Coordinate and develop referral pathways for dialysis vascular access.
• Maintain and audit CKD patient databases to support service planning and outcome evaluation.
• Monitor and report against key performance indicators.
• Support evidence-based nursing practice through literature review and guideline development.
• Contribute to quality improvement activities and nursing research initiatives.
• Provide expert clinical leadership and specialist advice within the multidisciplinary renal team.

The Renal Service provides specialist care to patients living with chronic kidney disease and those requiring dialysis therapies. Renal Services - North (Tas) - has an Acute, in-hospital, renal unit in the Launceston General Hospital, and an outpatient Satellite Renal Unit, in the Launceston Community Health Centre, 22 McHugh Street, Kings Meadows - which provides Haemodialysis and Renal Home Therapies, as well as chronic kidney disease education and preparation for dialysis.

• To be able to provide clinical nursing consultancy on issues of clinical practice, involving Chronic Kidney Disease and Renal Supportive Care and act as a role model and expert clinician within Renal Services - North.
• To be responsible for the effective planning, implementation and evaluation of a range of educational programs and activities for CKD patients and their families.
• To be able to promote the maintenance of effective communication channels between the members of the multi-disciplinary team, to support the CKD - education pathway for patients with Chronic Kidney Disease.

Selection Criteria

Clinical Leadership experience at a senior level in a tertiary, regional health service. or equivalent.

The Package

Based in Devonport, this multidisciplinary team provides intensive, home‑based mental health care for young people aged 16–24. The Position

Permanent full time, shift worker position, working 76 hours per fortnight, commencing as soon as possible.

Salary: $106,570 to $112,440 per annum. Employer 12% superannuation contribution is on top of this amount.

Be part of a multidisciplinary youth mental health service (16-25 years of age) delivering high quality mental health care and, in accordance with Mental Health Service principles, National Mental Health Standards, Agency policy, legal requirements and relevant professional competencies.

You will need to:

• Provide clinical expertise in contemporary nursing practice within the Y-HiTH setting, promoting professional standards of practice and an environment conducive to innovation and change that promotes best practice and meets established accreditation standards of practice.

The Child Health and Parenting Service is seeking a Lead Psychologist to join our new residential unit.

The Child Health and Parenting Service provides Primary Healthcare services to Families in Tasmania with children under the age of five, including health and development assessment, connection to other services, and a wide range of Parenting Support. The new Intensive Residential Parenting Unit will add to the current community support available, offering families the option of admission to a residential setting with focussed support for a wide range of parenting concerns, including parent and caregiver mental health and wellbeing. The unit is staffed by a nursing-led multidisciplinary team, working alongside the wider CHaPS network and with core connections with services across the Tasmanian service landscape, to deliver co-ordinated, comprehensive care in partnership with families.

As the Clinical Lead Psychologist within the Intensive Residential Parenting Unit, you will provide specialist psychological assessment, intervention and care coordination for parents and caregivers experiencing challenges to their mental health and wellbeing. Working as part of the multidisciplinary team, you will lead the planning and delivery of care for families, provide expert clinical advice, build strong partnerships with internal and external service providers, and contribute to the development of policies, programs and services that support positive outcomes for parents, infants and families. Peacock Centre is a contemporary mental health service, comprising of a twelve bed clinical inpatient unit (Peacock House) and a peer led Intergration Hub and Safe Haven, with care offered in a warm, welcoming environment. These services work closely together to provide least restrictive care for people experiencing suicidal distress and other mental health concerns and diagnoses as well as focussing on hospital avoidance principles. The successful incumbent must be able to work in a clinical environment but also be able to lead a peer led model of care. Referrals for the inpatient unit come from a number of different sources, and the unit must be able to respond appropriately in timely way; whilst the Safe Haven and the Integration Hubs are a "walk-in" environment with no referral required.

Peacock Centre, which is located in a heritage listed building, is an important part of Mental Health Services in Southern Tasmania. It is located in the centre of beautiful North Hobart, five minutes away from the centre of Hobart. There is ample parking (with a dedicated parking spot for the Manager). This role is expected to take part in an out of hours on call roster.

The role provides clinical oversight of the three services located in Peacock Centre (Peacock House, the Integration Hub and the Safe Haven). It also provides oversight of the fiscal and human resources that are required to run the Centre. The role must ensure that Peacock Centre is aligned within the model of care for the service, as well as ensuring it works collegially with other mental health services to ensure optimal operation and outcomes for consumers.

• Demonstrated knowledge of contemporary evidence based mental health service provision and extensive post-graduate clinical service delivery experience in the context of a multidisciplinary mental health team.
• Significant management experience in a community mental health setting and the capacity to provide leadership and vision to instil the principles and goals of Tasmanian Mental Health Reform Program and key documents specific to the Peacock Centre site.
• An understanding of, and capacity to implement, change management processes and strategies to assist in the initial establishment and the ongoing development and operation of the Acute Treatment Unit, Safe Haven, Integrated Hub and Recovery College Operational Service Models and related documentation.
