We're seeking a passionate AOD Counsellor / Case Manager to join our team in Longreach, supporting both adults and young people experiencing alcohol and other drug challenges through counselling, case management and recovery-focused support.

About the RoleYou'll deliver evidence-based alcohol and other drug treatment services, including:
- Assessments, counselling and case management.
- Treatment and recovery planning.
- Care coordination, advocacy and referrals.
- Collaboration with health and community service providers.
- Outreach services across the Central West region.
This role is full-time, Monday to Friday, 8:00am to 4:00pm, and is offered on a fixed-term basis until 30 June 2027. Subject to future funding arrangements, there may be an opportunity for the position to become ongoing.
Based in Longreach, you'll provide a combination of office-based and outreach service delivery. The role includes approximately one day of outreach travel per week to communities including Winton, Barcaldine and Blackall, as well as monthly overnight outreach visits to Jundah and Windorah.
A fully maintained Toyota Prado is provided for all work-related travel.
What We're Looking ForYou will have:
- Qualifications in Social Work, Psychology, Counselling, Human Services, Behavioural Science or a related discipline.
- Experience in counselling, case management or community service delivery.
- Strong communication, assessment and relationship-building skills.
- The ability to support people with complex needs using a person-centred approach.
- Experience working collaboratively with other services and stakeholders.
- Sound computer and administration skills.
- A current Australian Driver Licence.
- A current Blue Card, or the ability to obtain one.
Experience in alcohol and other drug services, outreach work, or rural and regional service delivery will be highly regarded.

This is an exciting opportunity to help shape an innovative service model designed to strengthen support before, during and after residential rehabilitation. As Team Leader you will provide leadership to a multidisciplinary team delivering care coordination, counselling, peer support, cultural support, transition support and post-treatment recovery services, helping people build sustainable pathways to recovery and wellbeing.
About the Tokoko Integrated Recovery Pathways ProgramThe Tokoko Integrated Recovery Pathways Program is a newly funded initiative designed to strengthen recovery outcomes for people accessing residential alcohol and other drug treatment in the Wide Bay region.
Working across Bundaberg and Hervey Bay, the program enhances the existing residential rehabilitation pathway through:
- Pre-admission engagement and treatment readiness support
- Alcohol and Other Drug (AOD) care coordination and navigation
- Transitional accommodation and recovery pathways
- Post-residential recovery and relapse prevention support
- Harm minimisation and psycho education.
- Peer and lived experience support
- Family and community engagement
- Enhanced referral pathways and service integration
- Digital recovery support through ConnectWell
The program aims to improve continuity of care, strengthen recovery outcomes and help people remain connected to support before, during and after treatment.
About the RoleAs the Team Leader - Programs, you will provide strategic, operational and clinical leadership across the Tokoko Integrated Recovery Pathways Program.
You will work closely with internal teams, community partners and stakeholders to support the establishment and delivery of integrated recovery-focused services throughout Bundaberg, Hervey Bay and the wider Wide Bay region.
Key responsibilities include:- Providing leadership, supervision and professional support to a multidisciplinary workforce.
- Promoting evidence-based, recovery-oriented and trauma-informed practice.
- Supporting effective risk management, care planning and continuity of care.
- Leading implementation and ongoing development of the program.
- Supporting workforce capability, quality improvement and service excellence.
- Building and maintaining strong relationships with health, housing, community and government stakeholders.
- Supporting culturally safe and inclusive service delivery.
- Contributing to program evaluation, reporting and sustainability activities.
This role is based in Bundaberg and will involve regular travel throughout the Wide Bay region.
What We're Looking ForYou will be a confident and collaborative leader who is passionate about supporting people affected by alcohol and other drug challenges.
To succeed in this role, you will bring:- Tertiary qualifications in Social Work, Psychology, Counselling, Nursing, Occupational Therapy, Human Services or a related discipline , and/or relevant experience.
- Demonstrated leadership experience within Alcohol and Other Drug (AOD), Mental Health or related human services settings.
- Experience providing supervision, mentoring or leadership to multidisciplinary teams.
- Strong understanding of recovery-oriented, trauma-informed and person-centred practice.
- Experience supporting people with complex needs and managing clinical risk.

Through ConnectWell, Lives Lived Well's digital wellbeing platform, you will help people stay connected to support throughout their recovery journey and improve access to services beyond traditional service settings.
About the Role
As the Clinician, you will play a key role in strengthening continuity of care for people engaging with alcohol and other drug treatment and recovery services.
Using ConnectWell, you will provide flexible, recovery-focused support that helps individuals remain connected throughout their treatment and recovery journey. This includes supporting engagement before admission, maintaining connection during treatment, providing follow-up support after discharge and assisting individuals to re-engage with services when additional support is needed.
Working collaboratively with the broader Tokoko Integrated Recovery Pathways team, you will help ensure people have access to timely support, information and service connections regardless of where they are in their recovery journey.
Key Responsibilities
- Deliver recovery-focused support and engagement through ConnectWell's digital platform.
- Conduct regular recovery check-ins with individuals preparing for, participating in or transitioning from residential rehabilitation.
- Support ongoing client engagement and continuity of care throughout treatment and recovery.
- Provide follow-up support for individuals following discharge from residential rehabilitation.
- Assist individuals to access appropriate alcohol and other drug, health, wellbeing and community support services.
- Support re-engagement for people who may be at risk of disengaging from treatment or recovery supports.
- Work collaboratively with the wider Tokoko Integrated Recovery Pathways team to strengthen coordinated care and recovery outcomes.
- Promote recovery-oriented, strengths-based and trauma-informed practice.
- Contribute to service development, client engagement initiatives and continuous improvement activities.
- Maintain accurate documentation, reporting and client records.
This role can be undertaken remotely, in a hybrid arrangement or from a Lives Lived Well office location, subject to operational requirements.
What We're Looking For
To succeed in this role, you will bring:
- Qualifications in Social Work, Psychology, Counselling, Occupational Therapy, Nursing, Human Services or a related discipline, and/or relevant experience.
- Experience working within the Alcohol and Other Drug, Mental Health or community services sector.
- Experience supporting engagement, recovery, care coordination, wellbeing or continuity-of-care initiatives.
- Strong understanding of recovery-oriented, trauma-informed and person-centred practice.
- Knowledge of alcohol and other drug treatment, recovery and harm minimisation principles.
- Ability to engage effectively with people experiencing complex needs.
- Strong communication, relationship building and engagement skills.
- Confidence using digital platforms and technology to engage and support clients.
- Ability to work independently while contributing positively within a multidisciplinary team.
- Current Australian Driver Licence.
Experience in telehealth, digital service delivery or online client engagement will be highly regarded.
