Good Shepherd Community Care - Named by The Boston Globe as one of the Top Places to Work in Massachusetts for the last four years, our community-based, mission-driven hospice and palliative care organization seeks an experienced and compassionate nurse practitioner to join our dynamic team in a full-time role. The Inpatient Palliative Care and Hospice Nurse Practitioner will join our expanding interdisciplinary palliative care service in an acute care setting to provide palliative care consultations and hospice admissions/discharges. This role is based at BMC Brighton.

• Provide recommendations about symptom management and facilitate goals of care conversations for seriously ill patients in medical, surgical, and ICU settings
• Participate in daily team rounds
• Communicate with care teams to facilitate high-quality, compassionate care
• Evaluate patients for hospice eligibility, conduct inpatient hospice (GIP) admissions, and facilitate discharges home with hospice
• Thoroughly document visits with adherence to billing practices
• Participate as a member of the hospital's interdisciplinary team, including representing palliative/hospice care in committees as applicable
• Conduct formal and informal education about palliative and end-of-life care

Salary: $110,000-$130,000
