About the role
The Clinical Nurse Consultant (CNC) Tissue Viability provides an advanced level wound assessment and management service as an expert in the specialty field and facilitates linkages and relationships with network hospitals (and affiliated services), nurses and medical officers (in the acute and community settings) to support the management of patients with acute, chronic and complex wounds.
The CNC is responsible for the provision of expert clinical advice, coordination, leadership and consultancy services within the wound assessment and management service and target populations within the designated network.
Responsibilities include:
- Development, implementation and evaluation of care management plans for patients with complex health needs.
- Providing leadership in the ongoing development of clinical practice.
- Initiating and utilising findings of research in the provision of clinical services.
- Contributing to the development and delivery of specialty related education programs
Participating in formal processes for the strategic and operational planning for the clinical service

About the role
Clarence Community & Allied Health have a temporary vacancy for a Clinical Nurse Specialist to join their team to provide care coordination, case management and support to people with chronic disease, their carers, General Practitioners, practice staff and staff involved in the provision of care. Working with relevant stake holders to ensure coordination of services, utilising shared care plans and other team care approaches to optimally support people with chronic disease within the framework of the Chronic Disease Management Service.
