About the role:
As a Personal Carer you’ll provide care to your clients living in the community, supporting their independence and improving their quality of life.
- Spending your time on the road completing scheduled visits, interacting one-on-one with clients within their homes supporting with daily activities
- In home personal care, daily living assistance including showering, personal hygiene, meal preparation, house cleaning, laundry and ironing
- Providing care in accordance with their individualised Care Plan
- Supporting with community based recreational and social activities

Role requirements:
- Certificate III Aged Care, Individual Support or an equivalent qualification - Essential

- Experience with technology incl. smartphones, tablets, laptops etc
- Confidence to satisfy the relevant probity checks required by legislation or policies

- Assist with meal preparation and service, ensuring meals are delivered safely and meet dietary requirements

- Perform cleaning and laundry duties, including maintaining resident rooms and shared areas and caring for clothing and linen

Role requirements:
- Prior experience in hospitality, cleaning, food services or similar environment
- Demonstrated ability to provide person-centred customer service with warmth and respect
- Strong communication skills and ability to work collaboratively in a team
- Ability to prioritise tasks and work effectively in a fast-paced environment
- Excellent attention to detail and commitment to hygiene and safety standards
- Understanding of or willingness to learn food safety, infection control and cleaning practices
- NDIS Workers Screening Clearance or willingness to obtain
- Confidence to satisfy the relevant probity checks required by legislation or policies

- Supporting and coordinating different teams within each neighbourhood
- Be the first point of contact for internal and external customers - via phone, email and in person

- Assist senior staff with special projects as required

- You will have effective verbal and written communication skills
- Previous experience in rostering/scheduling – Highly regarded

About the Role
Reporting to the Operations Manager, Residential Aged Care Services (RACS), the Deputy Operations Manager supports the effective operational management of SVHM Residential Aged Care Services, including Berengarra, Auburn House and Prague House. The role is responsible for assisting in the coordination of service delivery, workforce management, quality and safety systems, financial performance, and continuous improvement activities across the residential aged care portfolio. The Deputy Operations Manager (DOM) provides direct leadership and management to the RACS Lifestyle and Administration Teams and acts as the key liaison with Allied Health services to support coordinated, person-centred care and resident wellbeing outcomes. The role works collaboratively with Clinical Managers, the Director of Nursing, Business Manager, Allied Health professionals and broader multidisciplinary teams to ensure the delivery of safe, person-centred, high-quality care that meets all legislative, regulatory, accreditation and organisational requirements. The DOM acts on behalf of the Operations Manager as delegated and provides operational leadership to support service excellence, staff engagement, and achievement of strategic and operational objectives.
Classifications for this position will range from Grade 5 Allied Health OR for Nursing (ZB7) ($150,000 - 160,000 per annum plus super).
Your Contribution
• Support the Operations Manager in the day-to-day operational management of Residential Aged Care Services.
• Provide direct line management and oversight of the Lifestyle and Administration Teams, ensuring effective delivery of resident lifestyle and wellbeing programs and administration functions.
• Acts as the key liaison between the service and Allied Health (AH) teams, facilitating communication, coordination of care and collaborative service planning.
• Act on behalf of the Operations Manager as delegated, including participation in relevant meetings, committees and service reviews.
• Assist with the development, implementation and evaluation of operational plans and improvement initiatives.
• You will hold current AHPRA registration with the Nursing and Midwifery Board of Australia, without notations on the register, and a current NDIS Worker Screening Clearance, or be willing to undergo an NDIS Worker Screening Check.

Position Purpose

Operational Leadership and Service Delivery

- Build effective relationships with internal and external stakeholders.
- Support engagement with residents, families, advocates, regulatory agencies and community partners.
- Contribute to strategic planning, service development and organisational improvement.
- Foster collaborative relationships across Continuing Care, SVHM and external service providers.
About You
To be successful in this role, you will bring:
- A tertiary-level healthcare qualification.
- Current registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- Current satisfactory National Police and NDIS clearance checks.
- Previous operational management experience within a healthcare setting, preferably within aged care.
- Extensive clinical leadership experience and knowledge, preferably within residential aged care or a similar service.
- Highly developed leadership and organisational skills, with demonstrated experience leading multidisciplinary staff and functions.
- Highly developed written and verbal communication and interpersonal skills.
- An understanding of the Aged Care Act, Strengthened Aged Care Quality Standards and contemporary aged care reform agenda.
- Experience supporting quality improvement, accreditation, audit preparation and regulatory compliance.
- Demonstrated ability to build effective relationships and work collaboratively across multidisciplinary teams.

About the Role
The District Nurse Manager, Integrated Virtual Care Services (IVCS), provides strategic, operational and professional nursing leadership across a complex, district wide portfolio of virtual and community based services. About You

You can provide strategic, operational and professional leadership across multidisciplinary teams and complex service portfolios, with a focus on workforce capability, service performance, clinical governance and continuous improvement. You will need experience leading service redesign, organisational change and the implementation of innovative models of care, with the ability to translate strategic priorities into practical, sustainable improvements in service delivery. - Demonstrated ability to work with a high degree of autonomy and accountability while also working collaboratively in a multidisciplinary and multi service environment.

As the Paediatric Haemodialysis Clinical Nurse Educator (CNE) you will deliver and evaluate clinical education programs at Kidney Care Centre within Prince of Wales Hospital.
Some of the key functions of your role will be:
- Lead, coordinate, implement and evaluate evidence-based education programs that support the delivery of safe, high-quality paediatric kidney care across the Prince of Wales Hospital Kidney Care Centre and collaborating health services.
- Provide expert clinical leadership in paediatric haemodialysis, continuous kidney replacement therapies (CKRT), home dialysis therapies and specialised renal nursing practice.
As a CNE in SESLHD you will be recognised as a healthcare professional employing your clinical skills and critical thinking in delivering high quality patient care. You will exercise extended autonomy of decision making and professional knowledge and judgement in providing complex care requiring advanced clinical skills.
You will be an integral part of the interdisciplinary team in the provision of person-centred care; and lead, support and supervise other nurses and care roles. You will strive to ensure that the people who receive healthcare experience exceptional care that is person-centred, safe, high quality and compassionate.

Where you'll be working
Prince of Wales Hospital and Community Health Service is a Level 6 Tertiary Referral Hospital with an inpatient bed base of 370. Each year we care for more than 70,000 patients in our Emergency Department and have around 50,000 admissions to the inpatient units. POWH offers an outpatient service and rural outreach service and provides more than 900,000 occasions of non-admitted patient care each year, including innovative virtual models of care. We have transitioned the majority of inpatient units and wards to the Acute Services Building (ASB) including a state of the art Emergency Department and Intensive Care Unit. POWH provides an exceptional staff experience and enhanced patient and family outcomes as a result of the provision of person-centred care.

Selection Criteria
- Registered with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse.
- Current Certificate IV in Training and Assessment (TAE40122) or equivalent, or willingness to obtain within 12 months of commencement in role.
- Demonstrated extensive and recent clinical experience in paediatric nephrology and paediatric haemodialysis, including management of acute and chronic dialysis patients.

What you'll be doing
To provide senior clinical input, support, and supervision to the Community Mental Health Drug and Alcohol Service.
- Develop, facilitate implementation and evaluate care management plans for patients with complex health needs.
- Provide expert person-centred consultancy practice, participating in direct patient care provision and provide education on complex clinical issues to consumers and carers.
- Initiate and utilise findings of research in the provision of clinical services through initiation, conducting and dissemination of findings of locally based research in specialty.
- Participate in formal processes for the strategic and operational planning for the clinical service through identification of future needs and planning organisation, delivery and evaluation of specialist consultant service.

How to apply
To be considered for this position, please ensure you address the selection criteria in the application as thoroughly as possible:
- Relevant tertiary qualifications in nursing and current registration with the Australian Health Practitioner Regulation Agency (AHPRA)
- Minimum of five (5) years FTE post graduate MHDA care experience, preferably with post graduate qualifications relevant to MHDA service provision.
- Demonstrated commitment and ability to work collaboratively and effectively with MHDA consumers and carers.
- Demonstrated ability to synthesise information and develop clear and succinct reports/policy documents/Models of Care.
- Demonstrated ability to develop, implement, present and evaluate education/training.
- Evidence of understanding of and commitment to the consumer/care participation, the recovery model and person centred care.
- Demonstrated ability to work collegially with NGOs and colleagues to the benefit of consumers and carers.
- Demonstrated ability to implement changes and develop direction in clinical practice through planning, quality management and research
