As a Registered Nurse, you will play a pivotal role in coordinating and delivering evidence-based nursing care to older people with complex health needs. Working within a supportive and collaborative environment, you will utilise your clinical skills, critical thinking, and compassion to advocate for patients and contribute to their recovery, rehabilitation, and wellbeing.
What You'll Be Doing
·Delivering safe, high-quality nursing care to older adults with acute and complex health needs
·Undertaking comprehensive patient assessments and developing person-centred care plans
·Collaborating with medical, allied health, and nursing teams to optimise patient outcomes
·Supporting patients with dementia, delirium, frailty, and mobility challenges
·Providing education and support to patients, carers, and families
·Promoting patient safety, quality improvement, and evidence-based practice
·Mentoring and supporting junior staff and students within the clinical environment

A little about us.
The Acute Geriatric Unit provides specialised care for older patients with complex medical, cognitive, and functional needs. Our multidisciplinary team is committed to delivering person-centred care that promotes dignity, independence, wellbeing, and positive health outcomes for older people throughout their hospital journey.
Shoalhaven Memorial Hospital is the main referral hospital for the Shoalhaven region, providing emergency care, medical, surgical, mental health and orthopaedic services. The hospital has an Intensive Care Unit focusing on ventilated, high dependency and coronary patients, Children’s Ward and Maternity Unit, as well as a purpose-built regional Cancer Care Centre.
The hospital is currently undergoing a significant $440M redevelopment that will redefine how healthcare is delivered within the Region. Once complete in 2026, the new seven-storey Acute Services Building and refurbishments on the current hospital site will bring a focus on career development. allowing you to grow your career, acquire new skills and advance professionally as the hospital continues to develop. Within this redevelopment, a purpose-designed Mental Health Inpatient Unit will open for the first time at Shoalhaven Memorial Hospital, providing local consumers with access to comprehensive inpatient mental health care closer to home. There has never been a better time to join the progressive and expanding team in the Shoalhaven providing the highest standard of care to the Shoalhaven community.

The Acute Geriatric Unit provides specialised care for older patients with complex medical, cognitive, and functional needs. Our multidisciplinary team is committed to delivering person-centred care that promotes dignity, independence, wellbeing, and positive health outcomes for older people throughout their hospital journey.
Shoalhaven Memorial Hospital is the main referral hospital for the Shoalhaven region, providing emergency care, medical, surgical, mental health and orthopaedic services. The hospital has an Intensive Care Unit focusing on ventilated, high dependency and coronary patients, Children’s Ward and Maternity Unit, as well as a purpose-built regional Cancer Care Centre.
The hospital is currently undergoing a significant $440M redevelopment that will redefine how healthcare is delivered within the Region. Once complete in 2026, the new seven-storey Acute Services Building and refurbishments on the current hospital site will bring a focus on career development. allowing you to grow your career, acquire new skills and advance professionally as the hospital continues to develop. In this role, you will support young people, particularly those experiencing disadvantage or complex circumstances, to develop the practical, personal, social and emotional skills needed to live independently and achieve their goals.
Working within a strengths-based, trauma-informed and person-centred framework, you will facilitate engaging life skills activities that build confidence, resilience and community connection.
You will work collaboratively with Case Workers to ensure activities align with individual case plans and support goals across areas such as:
- Food preparation, nutrition and meal planning
- Budgeting and daily money management
- Household management and tenancy responsibilities
- Time management and establishing healthy routines
- Community participation and social connection
- Employment readiness, resume development and interview preparation
- Accessing housing, health, transport and social support services
You will also contribute to creating a safe, inclusive and supportive environment while maintaining professional boundaries, managing risk appropriately and accurately documenting client progress.

Your part in our UnitingCare team
We are currently seeking to appoint a Part-time Case Worker to work as a part of the Clare Homes Supported Accommodation Program.
Our Case Workers support families experiencing homelessness with strengths-based, trauma-informed housing case management whilst they are in short-term accommodation.
The position will provide outreach and mobile support to these families.
As part of this role, you will:
- Deliver rapid and coordinated assistance to families in critical need of housing and/or support through triage, strengths-based assessment, collaborative case planning and trauma-informed crisis intervention.
- Apply a strengths-based, person-centred and trauma-informed approach in all interactions, recognising the strengths, resilience and goals of each family and promoting their safety, choice and self-determination.
- Respond to and action referrals identified during case management with families, advocating on behalf of families and linking them with specialist and mainstream services through warm referrals.
- Adhere to program guidelines regarding the appropriate use of allocated funds.
- Undertake administrative duties related to the operation of the program, including maintaining clear, concise and accurate case notes and records within the appropriate timeframes, in line with privacy, confidentiality and duty-of-care obligations.
- Identify, assess and inform line management of current or potential risks that may impact on service delivery and the safety of clients and staff, and work with families to develop safety plans where required.
- Participate in regular supervision and reflective practice to support safe, ethical and trauma-informed practice, professional development and worker wellbeing.
- Positively represent the organisation when engaging with all stakeholders.
- Undertake other duties as and when required within the scope and capabilities of the role.
What makes a difference for us?
- A tertiary qualification (Diploma minimum) in social work, human services, community services or related field is essential
- Knowledge of, and demonstrated ability to apply, strengths-based, person-centred and trauma-informed practice with people experiencing or at risk of homelessness.
- Lived experience of homelessness, trauma or navigating the service system is highly valued, along with the ability to draw on that experience to build trust and inform respectful, person-centred practice. Applicants are not required to disclose personal experiences.
- You must have excellent verbal communication skills that enable you to assess needs and strengths accurately and extensively as well as transferable written communication skills to record assessments on the database.
- Ability to build rapport and actively engage in a respectful, non-judgemental way with people who have a range of needs, including people who have experienced trauma, and to provide culturally safe and responsive support, respecting the cultures of all people you support and work alongside.
- Ability to work collaboratively as part of a team as well as responsibly within an autonomous setting, and to reflect on and develop your practice through supervision.
- Demonstrated competence with technology, particularly Microsoft Office and tools for data entry.

Role requirements:
- Full, unrestricted AHPRA registration and unlimited work entitlements - Essential
- General nursing experience, post-registration
- Excellent communication and clinical problem-solving skills
- Current Australian Driver’s Licence (Green Provisional minimum) - Essential
- Roadworthy & insured (Third Party Property minimum) vehicle to use for work- Essential
- Confidence in managing your time effectively, including an ability to work autonomously.
- Confidence to satisfy the relevant probity checks required by legislation or policies
