As a Personal Carer you’ll provide care to your clients living in the community, supporting their independence and improving their quality of life.

Your part in our UnitingCare team
We are currently seeking to appoint a Part-time Case Worker to work as a part of the Clare Homes Supported Accommodation Program.
Our Case Workers support families experiencing homelessness with strengths-based, trauma-informed housing case management whilst they are in short-term accommodation.
The position will provide outreach and mobile support to these families.
As part of this role, you will:
- Deliver rapid and coordinated assistance to families in critical need of housing and/or support through triage, strengths-based assessment, collaborative case planning and trauma-informed crisis intervention.
- Apply a strengths-based, person-centred and trauma-informed approach in all interactions, recognising the strengths, resilience and goals of each family and promoting their safety, choice and self-determination.
- Respond to and action referrals identified during case management with families, advocating on behalf of families and linking them with specialist and mainstream services through warm referrals.
- Adhere to program guidelines regarding the appropriate use of allocated funds.
- Undertake administrative duties related to the operation of the program, including maintaining clear, concise and accurate case notes and records within the appropriate timeframes, in line with privacy, confidentiality and duty-of-care obligations.
- Identify, assess and inform line management of current or potential risks that may impact on service delivery and the safety of clients and staff, and work with families to develop safety plans where required.
- Participate in regular supervision and reflective practice to support safe, ethical and trauma-informed practice, professional development and worker wellbeing.
- Positively represent the organisation when engaging with all stakeholders.
- Undertake other duties as and when required within the scope and capabilities of the role.
What makes a difference for us?
- A tertiary qualification (Diploma minimum) in social work, human services, community services or related field is essential
- Knowledge of, and demonstrated ability to apply, strengths-based, person-centred and trauma-informed practice with people experiencing or at risk of homelessness.
- Lived experience of homelessness, trauma or navigating the service system is highly valued, along with the ability to draw on that experience to build trust and inform respectful, person-centred practice. Applicants are not required to disclose personal experiences.
- You must have excellent verbal communication skills that enable you to assess needs and strengths accurately and extensively as well as transferable written communication skills to record assessments on the database.
- Ability to build rapport and actively engage in a respectful, non-judgemental way with people who have a range of needs, including people who have experienced trauma, and to provide culturally safe and responsive support, respecting the cultures of all people you support and work alongside.
- Ability to work collaboratively as part of a team as well as responsibly within an autonomous setting, and to reflect on and develop your practice through supervision.
- Demonstrated competence with technology, particularly Microsoft Office and tools for data entry.

About You:
Our ideal candidate will demonstrate and to be successful in the role:

About the role
Preoperative Care
- Prepare patients for anaesthesia by completing assessments, reviewing medical history and allergies, and providing reassurance and education about the anaesthetic process.
- Ensure all anaesthetic equipment, monitoring devices, and emergency resources are checked, prepared, and functioning prior to each procedure.
- Safely prepare, label, and manage anaesthetic medications in accordance with hospital policies and clinical standards.
Intraoperative Care
- Assist the anaesthetist throughout surgical procedures by managing airway equipment, administering and handling medications as directed, and anticipating procedural requirements.
- Continuously monitor the patient's physiological status, recognising and responding promptly to changes in condition to support optimal patient outcomes.
- Contribute to the effective management of perioperative emergencies, including airway compromise, anaphylaxis, and cardiac arrest, while maintaining a calm and coordinated approach.
Postoperative Care
- Support safe patient recovery by assisting with extubating, ongoing assessment, and ensuring patients are stable for transfer to the recovery unit.
- Provide comprehensive clinical handover to the recovery team, including details of the anaesthetic provided, intraoperative events, and any postoperative considerations.
- Deliver effective postoperative pain management and monitor for complications or adverse effects, escalating concerns as appropriate.
About you
- Currently registered as a Registered Nurse (Division 1) with the Australian Health Professionals Regulation Agency (AHPRA)
- Demonstrated ability to work collaboratively within a multidisciplinary healthcare team, with excellent communication and interpersonal skills.
- A commitment to ongoing professional development and the application of evidence-based practice, critical thinking, and sound clinical judgement.

- The Renal Ambulatory Care (RAC) unit is made up of acute haemodialysis, satellite dialysis, peritoneal dialysis home training and renal clinics.
- Caring for inpatients and outpatients requiring renal replacement therapies - haemodialysis.
- Assess, plan, and deliver specialised nursing care for patients with kidney-related conditions, including those undergoing dialysis, by monitoring vital signs, fluid balance, and treatment responses.
- Educate and support patients and their families on managing chronic kidney disease, dialysis procedures, medications, and lifestyle adjustments, while collaborating with multidisciplinary teams to ensure optimal care outcomes.

The Medical Division at Coffs Harbour Health Campus provides comprehensive, patient-centred care across Medical A Ward, Medical B Ward and the Medical Assessment Unit, supporting more than 70 beds dedicated to patients with acute and complex medical conditions across a broad range of specialties. The Medical Assessment Unit offers a fast-paced environment focused on rapid assessment, treatment, stabilisation and discharge planning, providing valuable opportunities to develop advanced clinical assessment, critical thinking and multidisciplinary care coordination skills.
As a Registered Nurse, you will deliver evidence-based nursing care through comprehensive patient assessment, clinical decision-making, care planning, implementation and evaluation. Working collaboratively within a multidisciplinary team, you will coordinate patient care, respond to clinical deterioration, support patient flow and discharge planning, provide education to patients and families, and contribute to mentoring junior staff. You will also participate in quality improvement and professional development initiatives while promoting a positive, collaborative workplace culture.
This role operates within a seven-day rotating roster, including morning, afternoon and night shifts, and offers an excellent opportunity to further develop expertise in acute medicine, complex care, chronic disease management and multidisciplinary care coordination within a supportive and progressive healthcare environment.
