Role Requirements- providing skilled assessment and management of people with a life limiting illness
- participation in nurse led clinics and appropriate follow up and ongoing care/care coordination
- provision of clinical education, advice and support to other health professionals in the management of complex patients with specialist palliative care needs
- provision of direct patient care to community palliative care patients across a range of settings
- working as part of the team providing 7-day week services, with on-call duties as part of the rotating 7- day roster.
- leadership role in education, mentoring and working as a member of the multidisciplinary team
- working within Australian Nursing and Midwifery Board of Australia Standards of Practice and relevant codes that govern Registration, including accurate documentation.
- Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA) with relevant post-registration qualifications and/or at least 3 years' experience working in Specialist Palliative Care
- Demonstrated advanced clinical skills and ability to exercise autonomous decision making in providing complex care to palliative care patients
- Demonstrated high level communication and interpersonal skills
- Demonstrated application of evidence-based practice and critical thinking
- Demonstrated ability to provide mentorship and informal education to other health professionals
- Demonstrated commitment to provide culturally safe care to Aboriginal and Torres Strait Islander people
- Commitment to own professional development, maintaining competence and enhancement of skills
- Demonstrated high level written and verbal communication skills and information technology skills.
