The Role

The Team

The nurse unit Manager (NUM) is part of the Beaconsfield District health Service leadership team and works closely with the Director of Nursing to provide safe, high-quality person-centred care to our community.

The nursing team consists of Registered Nurses, Enrolled Nurses and Assistants, supported by allied health, hotel services, administration, activities and Diversional therapists, community and home help support staff. The team provides 24-hour care across aged care, sub acute, respite, palliative care, day centre and community services as part of a rural Multi purpose Service.

The team works collaboratively with a strong focus on resident and patient safety, quality improvement, workforce development and a positive, respectful culture. As part of the Tasmanian Health Service we partner with consumers, families, GPs and community organisations to deliver accessible integrated health care to the West Tamar community.

The Role

As Nurse Unit Manager at Beaconsfield District health Service, you will provide effective clinical and operational leadership across our rural Multi purpose Service. You will lead and support a multidisciplinary team in the delivery of safe, high-quality person-centred care across sub acute, residential aged care and community services.

You will be responsible for the day to day management of the service, including workforce planning, clinical governance, quality and safety, staff development and supporting positive workplace culture. Working collaboratively with staff, consumers, families and key stakeholders, you will contribute to the ongoing development and delivery of responsive health services that meet the need of the West Tamar community.

• Demonstrated extensive clinical knowledge and experience relevant to acute, subacute, emergency, Residential Aged Care, and community care.

The Team

Community Rapid Response Service - North (ComRRS-N) is a Nurse Practitioner led, General Practitioner (GP) referred hospital avoidance service within the Department of Health's Primary Health North. The team provides rapid assessment and short-term, hospital-level care for patients in their own homes or residential aged care homes helping to avoid hospital presentations and admissions within the Launceston region.

Our small experienced multidisciplinary team includes Nurse Practitioners, Registered Nurses and Allied Health Professionals, with backgrounds across intensive care, emergency, general practice, remote area nursing and primary health. We work collaboratively with GP's, hospital teams, Ambulance Tasmania and community providers to provide safe, coordinated, person-centred care. Operating seven days a week, we are committed to being trusted, responsive and flexible, providing the right care, in the right place, at the right time.

The Role

As a Nurse Practitioner within ComRRS-N, you will practise with a high degree of clinical autonomy, applying advanced assessment, diagnostic and treatment skills to deliver acute care in the community. You will collaborate with GP's, medical specialists, community nursing services, pharmacists and other healthcare providers to deliver coordinated, person-centred care. Within an experienced and supportive team, you will provide clinical leadership and oversight, supporting safe, evidence based practice and positive patient outcomes.

You will need

• Registration with Nursing and Midwifery Board of Australia as a Registered Nurse and endorsement as a Nurse Practitioner.
• Extensive clinical experience and advanced nursing expertise within a primary health care, community health or similar practice environment.

The Role

• The Registered Nurse is accountable to and works under the general direction and guidance of the Nurse Unit Manager/ Associate Nurse Unit Manager and receives guidance and support from other senior experienced Registered Nurses and health professionals for initiating, implementing, and evaluating quality nursing care.
• Involve patients/clients and their families/significant others in the planning and implementation of care.
• Work effectively within a multidisciplinary team, contributing to a strong team approach through open communication and a positive supportive approach.
