The Team

The Communicable Disease Prevention Unit (CDPU) comprises three divisions: Immunisation, Health Intelligence, and Response including Infection Prevention and Control. The primary purpose of the CDPU is to prevent and control communicable diseases within Tasmania. We actively engage with the public and collaborate closely with stakeholders to prevent and respond effectively to notifiable diseases.

The CDPU has delegated statutory responsibility for aspects of the Public Health Act 1997 and for the Poisons Regulations 2018 relating to immunisation. We receive notifications of diseases from laboratories and medical practitioners as stipulated in the Guidelines for Notifying Diseases and Food Contaminants.

The Role

As part of a multi-disciplinary team, the Lead Epidemiologist will provide surge capacity during outbreaks and public health emergencies by:

• Leading epidemiological investigations, risk assessments, data collection, analysis, interpretation and situational reporting.
• Developing and enhancing case management and contact tracing tools, and supporting the integration, management and quality assurance of data from multiple sources to inform timely public health action.
• Supporting business-as-usual communicable disease surveillance and epidemiology activities during periods of increased demand to maintain critical public health response and operational functions.

You will need

• Demonstrated wide experience in providing epidemiological support services, with advanced skills in conducting investigations and interpretating and evaluating data.
• Demonstrated experience in utilising statistical software packages (such as R and Stata) with the ability to extract information to produce quality reports.
• Proven ability to critically analyse and collate information, utilising professional judgement in the identification and resolution of problems, together with proven high level administrative, organisational and time management skills.

The Role

Everyone has a different definition of complexity, but for our team, complexity is very much about exceptional circumstances that are above and beyond usual practice for the health care teams working directly with the patient. This means that multi-medical or psycho-social presentations are not routinely considered complex by Complex Care.

Often our patients will have no clear hospital care pathways or sustainable discharge options back into the community, and many systems and services are involved that can lead to fragmentation of care and poor patient experience. The CCC team works with the patient, their supports, and the healthcare team to identify and coordinate care pathways, support patient flow, resolve barriers to discharge, avoid re-presentation to hospitals, reduce the burden of system pathway-finding on health care teams, and promotepartnerships with patients and successful working relationships with the healthcare team.

The Role

We are seeking an experienced allied health professional to join our team based predominately in the rehabilitation service at Launceston General Hospital, to work alongside nursing, medical and allied health teams, and in close collaboration with external service providers, to support the right place right care patient journey and quality outcomes for our patients.

The successful applicant will provide expert clinical advice on care management and systems pathways, complex discharge planning and hospital avoidance for patients with exceptional needs.

You will need

To be an experienced clinician who can confidently lead complex patient journeys, support high-risk discharge planning, and influence outcomes across multiple services, agencies, and stakeholders to achieve safe and sustainable patient pathways.

The Team

Community Rapid Response Service - North (ComRRS-N) is a Nurse Practitioner led, General Practitioner (GP) referred hospital avoidance service within the Department of Health's Primary Health North. The team provides rapid assessment and short-term, hospital-level care for patients in their own homes or residential aged care homes helping to avoid hospital presentations and admissions within the Launceston region.

Our small experienced multidisciplinary team includes Nurse Practitioners, Registered Nurses and Allied Health Professionals, with backgrounds across intensive care, emergency, general practice, remote area nursing and primary health. We work collaboratively with GP's, hospital teams, Ambulance Tasmania and community providers to provide safe, coordinated, person-centred care. Operating seven days a week, we are committed to being trusted, responsive and flexible, providing the right care, in the right place, at the right time.

The Role

As a Nurse Practitioner within ComRRS-N, you will practise with a high degree of clinical autonomy, applying advanced assessment, diagnostic and treatment skills to deliver acute care in the community. You will collaborate with GP's, medical specialists, community nursing services, pharmacists and other healthcare providers to deliver coordinated, person-centred care. Within an experienced and supportive team, you will provide clinical leadership and oversight, supporting safe, evidence based practice and positive patient outcomes.

The Role

Career Medical Officer - Psychiatry - Mental Health Emergency Department.

The Department is seeking a suitably qualified Career Medical Officer (CMO) - Psychiatry Day Work - Mental Health Emergency Department to provide psychiatric assessment, management, and clinical care for consumers presenting with acute mental health concerns.

This is a dedicated psychiatry position working within a specialist mental health emergency service at the Royal Hobart Hospital. The role requires a clinician who is confident in the assessment and management of psychiatric emergencies and capable of making sound clinical decisions.

The successful applicant will work with a high degree of professional autonomy and limited on-site medical supervision, while having access to established escalation pathways and specialist psychiatric consultant support as required.

You will provide mental health medical services across the Royal Hobart Hospital, including:

• Providing medical care to patients in inpatient mental health units, including the Short Stay Unit.
• Attending to mental health patients located throughout the hospital who require specialist medical input.
• You may also be allocated additional duties consistent with your qualifications and clinical experience.

While performing this role, you will be expected to:

• Liaise closely and professionally with consultant psychiatrists regarding patient assessment and management.
• Maintain accurate, timely, and comprehensive clinical documentation, including admissions, progress notes, decisions, and discharge summaries
• Ensure all medical decisions are clearly recorded in the patient medical record
• Promote collaborative working relationships with the multidisciplinary team involved in patient care
• Respond promptly to clinical calls from Registered Nurses and other clinical staff
• Perform clinical procedures as required and within scope of practice
• Ensure accurate prescribing, administration, and documentation of medications and treatments
• Actively participate in discharge planning and continuity of care
• Comply with all professional, ethical, and legal obligations, including requirements under the Tasmanian Mental Health Act
