Your opportunity
Reporting to the Senior Tailored Support Coordinator, you'll assess resident needs and work collaboratively with each person to develop practical, achievable support plans.
You'll connect residents with Anglicare services, community providers and specialist supports, while coordinating services and monitoring progress to help achieve stronger social, health, recovery and wellbeing outcomes.
With integrity and resilience, you will:
Complete resident needs assessments and develop tailored support plans with clear, achievable outcomes.
Connect residents with health, legal, financial, advocacy, community and Anglicare services.
Coordinate case discussions and build productive relationships with residents, families and service providers.
Monitor support plans, resident progress, service quality and agreed budgets, responding when needs or circumstances change.
Maintain accurate and confidential case records and contribute to wellbeing, participation and tenant satisfaction reporting.
What you'll bring
You're a compassionate and organised practitioner who knows how to listen, assess individual needs and coordinate the right supports to help people achieve meaningful outcomes.
You'll bring strong case coordination and relationship-building skills, together with an understanding of how housing, culture, crisis and complex needs can shape a person's experience.
You also share our organisational values of integrity, justice, compassion and excellence, embedding them in everything you do.
What's required:
Qualification in Social Work, Community Services, Allied Health, Aged Care, Community Housing or a related field, and/or relevant experience.
At least 12 months post-qualifying experience delivering community-based social services, including assessment and care coordination.
Experience coordinating services for people with complex needs and maintaining accurate case documentation.
Strong communication, advocacy and stakeholder relationship skills.
Current driver licence and ability to meet the role's required probity and compliance checks.
What's preferred:
Experience supporting residents within social or affordable housing, aged care or community services.
Understanding of evidence-based, culturally safe and trauma-informed practice.
Knowledge of community services, referral pathways and multidisciplinary support networks.

About the role
Working in the Heart Lung Stream, providing social work services to inpatients, families, and carers within Cardiology , Cardio-Thoracic Surgery, Thoracic Medicine, Thoracic and Cardiac Rehabilitation and Heart and Lung Transplant including psychosocial assessment, risk assessment, counselling, and family support
Participate in multidisciplinary team meetings to support patient care, discharge planning, and coordination of referrals to community supports, accommodation, and placement services
Advocate for patients and carers, including support with NCAT Guardianship and NDIS applications where required
Contribute to clinical operations including documentation, handovers, statistical reporting, clinical cover, on-call duties, and resource development
Engage in professional development activities including supervision, education, research, student training, quality improvement, and mandatory training compliance
About you
Hold a degree in Social Work and be eligible for membership with the Australian Association of Social Workers (AASW), with demonstrated clinical experience in a tertiary hospital or health-related setting
Strong clinical skills and knowledge in psychosocial assessment, counselling, therapeutic interventions, case management, advocacy, and complex discharge planning
Understanding of and practical experience in crisis intervention, short-term solution-focused approaches, and grief and loss support
Ability to work effectively within multidisciplinary teams in high-pressure environments, managing competing demands with strong negotiation and liaison capabilities
Willingness and ability to participate in the after-hours on-call hospital social work crisis service as required

About the opportunity
The AOD clinician will work collaboratively with the HOPE team to actively engage clients in the assessment of their own suicide risk and actively contribute to their suicide prevention plan in order to minimize the risk of attempting suicide in the future.
The AOD clinician will assist the HOPE team in responding to new referrals in a timely manner, work with consumers and their families and carry out discipline specific duties and assessments. The HOPE team will work in a manner that encourages and invites consumer participation and promotes self-determination. The role includes a component of outreach work, and as such, some weekend and afterhours work will be required.
Qualifications:
- Registration with AHPRA
-as a Registered Nurse with tertiary qualification in mental health nursing or
-as a Psychologist with endorsement as a Clinical Psychologist or
-as an Occupational Therapist or
-a recognized degree in social work with eligibility for registration with Australian Association of Social Workers
- Minimum of 5 years post graduate experience including having worked within a community mental health setting.
- Completed Cert IV in AOD training or higher.
- Extensive experience in managing client with AOD issues.
- A current unrestricted Victorian Drivers Licence is essential.

- Support the recruitment, assessment, induction, training and ongoing development of Professional Foster Carers.

- Develop and review Professional Foster Care Plans and therapeutic support strategies that promote positive outcomes and placement stability.

- Coach and support carers to implement therapeutic, relationship-based approaches within the home environment.
- Facilitate care team meetings, case reviews and collaborative planning with key stakeholders.

- Maintain accurate case notes, reports and compliance documentation in line with legislative and organisational requirements.
- Identify and respond to safeguarding, risk and placement concerns, contributing to effective support and risk mitigation strategies.
- Participate in supervision, reflective practice, professional development and after-hours on-call arrangements as required.

- Bachelor's Degree in Social Work, Psychology, or a recognised equivalent qualification in a relevant human services discipline.

About the Role
Practicing within the Strengths Model of Recovery Oriented Care, under the guidance of the Team Leader, Consultant Psychiatrist, and in conjunction with the Program Manager Early Psychosis (St Vincent’s Mental Health), the Early Psychosis Senior Clinician is responsible for the implementation and maintenance of the Early Psychosis Program (EPP) at Hawthorn Community Mental Health Clinic. EPP is a clinical stream that delivers early intervention to consumers who are at risk of developing a psychotic illness or experiencing a first episode of psychosis who may also present with a range of co-morbid mental health issues.
Ensure service provision is guided by inclusive practices for all young people and families of all backgrounds and incorporates culturally safe and trauma informed principles
Provide comprehensive bio psychosocial assessment, direct care, and consultation for consumers, families and significant others within an assertive, intensive case management model
Provide secondary consultation to case managers within the service to ensure the best outcomes for all
Classifications for this position will range from:
OT/SW Grade 4 HR9 - HR12 or HR25 - HR28 ($107,260 - $122,548 per annum pro rata)
RPN Grade 4 NP75 - NP77 ($121,274 - $130,603 per annum pro rata)
Your Contribution
A minimum of 5 years post qualification in psychology, a minimum of 5 years post qualification in nursing, and a minimum of 7 years post qualification in Occupational Therapy or Social Work.
Proven ability to provide high quality mental health interventions to people with serious mental illness, including assessment, treatment, case management, crisis intervention, rehabilitation, demonstrated skills in engaging consumers with complex needs and a capacity to identify and respond to dual diagnosis clients
A sound knowledge of current early psychosis theory and practice within public mental health.

We are seeking applications for a Psychologist or Accredited Mental Health Social Worker (grade 2) to join our Mental Wellbeing Service on a permanent part-time basis in Ballarat, Victoria.
The Grade 2 clinician is a member of a multi-disciplinary team that provides evidenced-based, person centred, trauma informed mental health care to the clients of St John of God Health Care (SJGHC) Mental Wellbeing Service.
You will be responsible for the assessment and treatment planning of an individual client caseload, including thorough risk assessments. You will also be working as part of a multidisciplinary team. Our program is also open to the development and implementation innovative group programs for our client group. Our consumer group include those with mild to moderate mental health conditions.
All applicants must be eligible for a Medicare Provider Number to enable access to the Better Access to Mental Health Scheme.
The Position
Provides advanced level psychological services including a range of individualised and group clinical care based on professional training reflecting current ethical clinical practice as required.
Provides comprehensive assessment and care planning monitoring consumer progress adjusting treatment accordingly for complex management.
Liaise with multi-disciplinary team to promote effective communication and treatment planning.
Familiar and complies with legal, ethical responsibilities to consumers, visitors and staff, inclusive of maintaining confidentiality.
You will have experience providing group programs and individual therapy to adult consumers presenting with a myriad of concerns, demonstrate clinical leadership skills and ability to undertake quality improvement activities.
Successful applicants will have tertiary qualifications in Psychology or Social Work and current registration with the Psychology Board of Australia or AASW. Experience running groups and providing individual therapy using CBT, DBT and Schema-Focused Therapy will be highly regarded.

About the role

Your part in our UnitingCare team
The Aspire program, based in Townsville, provides free support to young people who are not being protected by their family, friends and communities.
As Case Manager with the Aspire Program in Townsville, you will facilitate the development of quality practices and effective coordination of the service delivery, including group programs, to vulnerable young people accessing the service. In this role, you will work with youth to build capacity, connectedness, reliance, wellbeing, education outcomes, career options, safe and stable housing and positive family and community relationships.
The key responsibilities include but not limited to:
- Intake assessment
- Client centred planning.
- Counselling and therapeutic support
- Case Management
- Integrated Responses in partnership with other stakeholders in support of clients of Aspire.
What makes a difference for us?
- A minimum of an undergraduate qualification in a relevant human services field i.e. Social Work, Counselling, Psychology or other Human and Behavioural Science discipline
- Sound knowledge of and demonstrated ability to utilise therapeutic processes and practices including the ability to analyse, evaluate and make objective judgments and recommendations.

About the role

About Us
Monash Newborn is a 64 bed medical and surgical neonatal unit, with a reputation for delivering outstanding family centred care to neonates. Monash Newborn enjoys the many benefits of being co-located with the tertiary paediatric services at Monash Children’s Hospital, and the tertiary Maternity services at Monash Medical Centre. With the support of a broad range of paediatric sub specialists, almost all neonatal medical and surgical problems can be managed ‘in-house’ at Monash Newborn, with the exception only of open-heart surgery, ECMO, and major metabolic disorders. Having a premature or unwell baby in Monash Newborn places an enormous amount of stress on families. To ease the pressure on families and to make beds available for other infants in need, medically stable infants are down-transferred to a hospital closer to home as soon as possible.
Monash Newborn is overseen by two Nurse Managers, and 14 EFT of skilled and experienced Associate Nurse Managers (ANMs). In partnership with the Nurse Managers, the ANMs lead the team of Nurses who are committed to providing an excellent standard of health care to the newborn infant and their family. The Monash Newborn medical team consists of a team of Neonatal Consultants, Fellows, and NMOs. The medical team provide a round-the-clock in-house presence of staff, who are experienced in neonatal care. Monash Newborn has the support of a broad Multidisciplinary Allied Health team, including social work, physiotherapy, speech pathology, pharmacy, music therapy, occupational therapy and biomedical engineering.

About the Role
We are looking for a Grade 1 Social Worker to join our Adult Acute Social Work team at Casey Hospital in a full-time position.
This is an exciting opportunity to build your clinical skills in a busy acute hospital environment while being surrounded by an experienced, supportive and collaborative Social Work team. About the Role:
As a Social Worker within the Northern Adelaide Local Health Network (NALHN) Mental Health Services, you will play a vital role in delivering high-quality, person-centred care to consumers accessing community-based mental health services. You will provide comprehensive assessment, intervention and care coordination to support recovery and optimise health and wellbeing outcomes for individuals, their families and carers.
At the AHP1 classification you will develop your clinical practice and professional capabilities while working under direct clinical supervision. You will:
- Deliver social work services within a mental health context, including assessment, care planning, intervention and discharge planning
- Apply developing knowledge and skills to support consumers with a range of mental health presentations
- Work collaboratively within multidisciplinary mental health teams to provide coordinated care

At the AHP2 classification, you will apply your clinical experience with greater autonomy to deliver high-quality, evidence-based social work services.

- Manage a caseload with reduced supervision, including non-routine and complex clinical presentations
- Demonstrate sound clinical reasoning and professional judgement in a mental health setting
- Support and guide less experienced staff, students and allied health assistants

About You:

- A recognised Social Work qualification with eligibility for full membership of the Australian Association of Social Workers

About the Role:

About the Role:
Join SA Health's Northern Adelaide Local Health Network as a Social Worker in our Casual Pool for 2026-2027 with potential to move to contracted hours of work. Based at Lyell McEwin and Modbury Public Hospitals, you will provide essential social work 7-day rostered services across various clinical departments, supporting patients and families during challenging times, including Domestic, Family Violence, Child Protection, Elder Abuse, High Risk Pregnancy assessments. This casual position offers flexibility while contributing to South Australia's premier public health system.
What You'll Do
- Conduct brief crisis comprehensive bio psychosocial assessments and develop intervention plans
- Write professional reports and discharge plans
- Provide brief crisis counselling and support services to patients and their families
- Collaborate with multidisciplinary teams to ensure holistic timely safe patient care
- Facilitate discharge planning and community service referrals
- Maintain accurate documentation and case records
What We're Looking For
- Experience in healthcare or hospital settings preferred
- Strong critical thinking and problem solving, clear communication, writing and interpersonal skills
- Ability to work under instruction but also independently and manage competing priorities
- Be willing to follow infection control procedures including being Bare Below the Elbows on the hospital wards.

About the role
Join the Northern Country CAMHS team as a Social Worker or Senior Social Worker, delivering high-quality, evidence-based mental health services to children and young people aged 0–16 and their families across regional South Australia. You will provide comprehensive assessment, therapeutic interventions, education, counselling, group work, and consultation within a multidisciplinary and interagency framework. Both roles accept accountability for the outcomes of clinical practice, ensuring optimal consumer care and recovery, and are responsible for addressing any inconsistencies between practice and policy. The Senior Social Worker role additionally provides leadership through supervision, direction, support, and education of staff, as well as participation in action research projects, quality improvement initiatives, and policy development. Both roles promote culturally responsive, person-centred care, contribute to service planning, and foster ongoing professional development and excellence in clinical practice.
Essential Qualification:
A degree level qualification in Social Work which gives eligibility for full membership of the Australian Association of Social Workers.
