The Role
This position supports Local Court Magistrate with the diversion of defendants who have or are suspected of having a mental illness, mental disorder or a cognitive impairment from the Criminal Justice System (Court and Correctional Centres) into the community.
Candidate must hold a relevant post graduate qualification in mental health and a current NSW Drivers Licence.
Clinical Nurse Consultant Grade 2
- Current authority to practice as a Registered Nurse with the Nursing & Midwifery Board of Australia.
- Current full registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- Five years full time equivalent post registration clinical experience, with at least 3 years full time equivalent in an acute mental health setting.
Occupational Therapist Level 4:
- Must hold qualifications recognised for registration with the Occupational Therapy Board of Australia.
- Must hold general registration with the Occupational Therapy Board of Australia.
- Five years full time equivalent post registration clinical experience, with at least 3 years full time equivalent in an acute mental health setting.
Social Worker Level 4:
- Minimum Bachelor’s degree in social work which provides eligibility for membership of the Australian Association of Social Workers, or other qualification deemed equivalent by the employer.
- Five years full time equivalent post registration clinical experience, with at least 3 years full time equivalent in an acute mental health setting.
Clinical Psychologist:
- Four year degree in psychology, being a three year degree with a fourth year honours in psychology; or a qualification deemed equivalent by the employer.
- Hold general registration with Australian Health Practitioner Regulation Agency (AHPRA).
- Five years full time equivalent post registration clinical experience, with at least 3 years full time equivalent in an acute mental health setting.

About the ED Mental Health & AOD Response at Monash Health (EMAAP- Emergency Mental Health, AOD, Allied, Peer Team)
The MMC Emergency Department has an inter professional team which brings together clinicians from mental health, alcohol and other drug (AOD), Social Work and Lived Experience to provide specialised and integrated care to patients presenting to the ED with high acuity mental health and AOD issues, and complex psychosocial needs. This service is called the Mental Health and AOD Response or EMAAP and is located within the emergency department. The team will provide a seven day a week, extended hours service as well as outreach community follow up and support for a 28-day period post patient discharge.
About The Role
This role is a full-time ongoing opportunity, based and Monash Clayton Emergency Department.
The Alcohol and Other Drug (AOD) Clinical Nurse Consultant (CNC) role is a fantastic opportunity to join the team in the ED, enhancing the quality of care for people presenting to the Clayton Emergency Department with mental health or behavioural health issues through clinical leadership, collaboration, consultation and support of ED clinicians. Working at an advanced nursing practice level, you'll provide evidence-based, person-centred clinical leadership and expertise, and be accountable for your own clinical practice while providing support and direction to the inter-disciplinary team.
- Build strong, collaborative relationships with ED medical, nursing, mental health, addiction medicine, peer support and allied health staff, providing education and coordination to ensure effective care for patients with substance misuse issues.
- Establish supportive relationships with patients presenting with an AOD or dual diagnosis, and their families, carers and support networks.
- Conduct advanced, holistic AOD assessments — covering biopsychosocial factors, physiological indicators and collateral information from family, carers and GPs, and develop collaborative care plans and therapeutic interventions to reduce substance-related harm.
- Provide assertive outreach and a small, person-centred caseload for up to 28 days post-discharge, connecting consumers with community supports consistent with their goals.
- Develop and maintain referral pathways to counselling, outpatient clinics and community agencies to support timely discharge and ongoing care.
- Embed recovery-oriented practice, promoting choice and self-determination within medico-legal requirements and duty of care.
About You
You are an experienced nurse who brings:
- A postgraduate qualification in AOD specific studies and current registration with AHPRA
- Advanced AOD clinical assessment skills, with experience in acute health settings and/or the emergency department
- Excellent communication skills and proven leadership qualities
- A passion for improving the experience of care for people in crisis, delivering services that respect the individual's rights, preserve their dignity, and promote recovery and inclusion

Your opportunity
Working alongside our Residential Aged Care clinical teams and Chaplains and Pastoral Carers, you'll provide professional counselling and support to residents and families experiencing grief and bereavement.
You'll work collaboratively with residents and families to understand their individual needs, develop achievable support plans and facilitate the grieving process with empathy and sensitivity.
Your impact will extend beyond individual counselling. You'll also support Residential Aged Care employees to understand their own vulnerability when working with grief and loss, promoting practical self-care strategies and strengthening the capability of our teams to respond compassionately to bereavement.
With integrity and compassion, you will:
· Provide specialist grief and bereavement counselling and support to residents and their families.
· Advocate for residents and families while acknowledging and facilitating their individual grieving process.
· Develop individual case plans with residents and families, establishing clear and achievable outcomes.
· Provide education and support to Residential Aged Care employees around grief, vulnerability and self-care.
· Identify complex grief and establish strong external referral pathways to ensure people can access specialist support when required.
· Develop resources that promote awareness and accessibility of grief and bereavement support across Residential Aged Care Homes.
· Maintain accurate client documentation and statistical information using appropriate evidence-based tools.
· Contribute to continuous improvement of the service through professional development, policies, procedures and work practices.

About the opportunity
We are looking for experienced RPN Grade 3's or equivalent to join Mercy Mental Health Casual Bank to cover planned leave and short notice vacancies within the CATT and EMH teams.
- The role of the CATT Clinician is to provide intensive clinical assessment, treatment and support services to known clients who are in acute psychiatric crisis or at risk of relapse.
- The EMH Clinician provides intensive clinical assessment, treatment and support services to clients who are in acute psychiatric crisis who present to Werribee Mercy Hospital ED, PACER or the MMH Triage service.

Qualification/ registration requirements
- Registration with AHPRA as a Registered Nurse with successful completion of a specialist undergraduate psychiatric nursing program or a Post Graduate Psychiatric/mental health nursing qualification or
- An approved degree in Psychology and registration with AHPRA as a Psychologist or
- Registration with AHPRA as an Occupational Therapist or
- A recognised degree in Social Work and eligibility for membership of the Australian Association of Social Workers.

About the Role
The Disability Access and Inclusion Plan (DAIP) Project Lead will coordinate, monitor and report on the implementation of LCLHN’s Disability Access and Inclusion Plan and Autism Action Plan (AAP), ensuring commitments progress within established timeframes.

The Southcare Outreach Service (SOS) provides multidisciplinary care in the community for the ageing population over 65 years of age residing in the Sutherland Shire. The SOS provides short term acute interventions (up to six weeks) by Nursing, Physiotherapy, Occupational Therapy, Social Work, and our consulting Geriatrician to facilitate enhanced patient care and safe clinical outcomes. SOS staff will provide assessment, intervention and referral onto other required services in order to navigate and integrate the patient’s journey between tertiary and primary care.
The Southcare Outreach Service (SOS) provides a 7 day a week service including public holiday coverage, with varied shift times (a mixture of 8hr and 10hr shifts).

About the role:
Reporting to the Manager, this role is responsible for provide specialist social work assessment, intervention and care planning to people with complex medical, functional and psychosocial needs — supporting them to remain in their own home in lieu of an extended or avoidable hospital admission.

About you:

Ideal Candidate

Your Role as Administration and Intake Officer
As an Administration Coordinator, you’ll support The Way Back and Suicide Prevention Services by keeping operations running smoothly, managing referrals, conducting intake screenings, and acting as the first point of contact to help deliver timely, high-quality care. Key accountabilities include:
- Maintain program compliance, audits, and accreditation
- Keep credentialing, registrations, insurance, and MOUs current
- Liaise with internal and external stakeholders
- Prepare weekly invoices in TechOne
- Provide reception and frontline admin support
- Process referrals and conduct intake and follow-ups
- Coordinate staff induction and orientation
- Maintain IT services
- Reconcile WMQ credit cards
- Assist with events and meetings
What We’re Looking For
- NDIS Worker Screening Clearance and Orientation completed
- Relevant qualification and/or experience in admin or community services
- Strong admin and IT systems skills (Office 365, TechOne, databases)
- High-level interpersonal and communication skills

- Strong problem-solving and analytical ability

Your part in our UnitingCare team
The Intensive Family Support Service assists vulnerable families with complex needs who are at risk of entering the child protection system. Our voluntary service supports families in their homes, generally through weekly visits for up to 12 months. This enables us to work collaboratively with families to keep children safe and achieve meaningful family goals.
As a Child and Family Case Worker, you will lead case management with families in a fast-paced, high-volume environment, helping them identify and prioritise their concerns.
This role involves:
- Providing a combination of therapeutic support and practical in-home assistance to vulnerable families with complex needs who may be difficult to engage.
- Actively engaging with families, completing assessments and developing collaborative case plans that prioritise their presenting concerns.
- Supporting families to access appropriate specialist services
What makes a difference for us?
- A bachelor’s degree or higher in social work, human services, psychology or a related discipline is preferred. Applicants with a diploma in a related field and at least two years’ experience working as a Case Worker will also be considered.
- At least two years’ experience in case management and child protection, working directly with families, children and young people with complex needs.
- Current knowledge and understanding of domestic and family violence and its impact on children.
- An understanding of First Nations peoples, cultures and communities, including the issues that may affect them.
- A commitment to culturally safe practice.
- Sound knowledge and understanding of attachment theory, trauma-informed practice and child development.
- A current Queensland Working with Children Check, or Blue Card.

About the Service
The Family Care Midwifery Team provides continuity of antenatal and postnatal care to pregnant people living with psychosocial vulnerabilities. You will support families from early pregnancy until 4–6 weeks post birth, working collaboratively to provide trauma-informed, respectful, and culturally safe care.
This is a small, supportive team of midwives committed to delivering high-quality care to vulnerable families. You will work closely with a range of services including obstetrics, drug and alcohol services, social work, mental health teams, and other allied health professionals. The team culture is collaborative, flexible, and focused on providing compassionate, safe, and client-centred care.
About the Role
As a member of the Family Care Midwifery Team, you will provide continuity of care for vulnerable families, including both antenatal and postnatal care. Your day-to-day activities may include:
- Providing care to women and newborns in hospital and community settings
- Attending case conferences and multidisciplinary meetings
- Making appropriate referrals to enhance client care
- Collaborating with allied health, social work, and mental health teams
- Managing the diverse and unpredictable needs of families within a continuity of care model
You will be supported by a team of experienced midwives and will have access to monthly clinical supervision and ongoing education opportunities to develop your skills. Flexibility, strong time management, and the ability to work independently while collaborating effectively with others are essential for success in this role.

The Perinatal and Infant Mental Health Team offers specialist perinatal and parent-infant mental healthcare. Open Support is a community service organisation committed to providing assistance to families in need across New South Wales. Open Support offers services that are professional, flexible and responsive to emerging community needs.
As one of our key service areas, we provide crisis accommodation, transitional accommodation support, case management and child specific case support to CALD and migrant women and children who have experienced and are escaping family violence. In addition to our crisis facility, support is also offered to clients who are transitioning or have transitioned from crisis accommodation to longer-term housing and in an outreach capacity.
The Open Support DFV Program aims to ensure that women and children are supported in ways that leave them empowered to make choices that prioritise their own safety & wellbeing and that lead them to experience and increased sense of control and hope for their futures.

The Role
An exciting opportunity exists for a motivated early childhood professional to lead the development and implementation of a child and family support framework for Open Support Domestic & Family Violence Service. Your knowledge and experience in working with children and families who have experienced trauma will help to guide service delivery and drive the provision of high-quality, child centered, trauma informed, support.
This role will allow you to utilise both your practical skills in working directly with children and families as well as your program and project management skills, to ensure that best practice is established and embedded into the service delivery framework at Open Support. Your experience in developing and delivering individual and group interventions will be crucial to your success in this role.
You will also have a sound working knowledge of relevant standards and guidelines in relation to child protection, including the application of and compliance with the National Principles for Child Safe Organisations.
Essential Criteria
- Minimum Tertiary qualification in Early Childhood Education, Social Work, Psychology, Occupational Therapy or similar qualification in related discipline
- Experience supporting children and/or families affected by DFV to realise positive change, using a flexible, child centred and trauma informed approaches to service delivery.
- Demonstrated understanding of early childhood development and experience in developing and applying play based approaches within a family violence (or related) setting.
- Knowledge of specific issues facing children who have experience trauma and its impact on child development, attachment and behaviour.
- Demonstrate ability to create, implement and evaluate evidence based programs of support
- Ability to identify and effectively manage risk as it relates to children and families in a DFV setting
- Demonstrated ability to communicate and network with multiple service providers to ensure client’s needs are met.
- Strong interpersonal, active listening and negotiation skills and experience in effectively managing difficult situations.
- Demonstrated skills in working both autonomously and within a small team context
- Knowledge of some of the specific issues facing migrants and people from CALD backgrounds who experience DFV.
- Completion of ECAV training or other approved child protections qualification, or willingness to work toward same.
- Current NSW Drivers Licence, Working With Children Check and NSW Police Check clearance
- Current First Aid Certificate & Mental Health First Aid Certificate or willingness to work toward same.
- Willingness to participate in an on call roster 1 week every 5
Desirable Criteria
- Ability to speak a second language (other than English).
- Experience in facilitating groups with children and families
- Experience in using web-based client management systems
