About the role:
As a Senior Personal Carer you'll conduct initial client intake and screening assessments and developing and reviewing individualised care plans with a client-centered approach while providing quality care based on clients' needs.
- Conduct thorough initial screening assessments, developing care plans for personal self-care, general house cleaning, social support, respite, transport and shopping assistance.
- Management of client information in CRM & CMS ensuring all details and case notes are kept up to date
- Engage in ongoing care discussions, planning, assessment, reviews, and evaluating goals and outcomes in consultation with the care team and as required.
- Spending your time on the road completing scheduled visits, interacting one-on-one with clients within their homes supporting with daily activities
- Providing care in accordance with their individualised Care Plan
- Supporting with community based recreational and social activities
- Transportation and assistance with shopping and appointments.

Role requirements:
- Certificate III Aged Care, Individual Support or an equivalent qualification - Essential
- Current Australian Driver's Licence (Green Provisional minimum) - Essential
- Roadworthy & insured (Third Party Property minimum) vehicle to use for work- Essential
- Experience with technology incl. smartphones, tablets, laptops etc
- Confidence to satisfy the relevant probity checks required by legislation or policies

As a valued Home Care Worker at Anglicare Woolooware Shores based in Taren Point on site working on a Casual basis, a vibrant and welcoming community set in the heart of Sydney’s Southern suburbs.
You will provide compassionate, high-quality support to clients in their own homes, helping them maintain independence and live with dignity.
You will assist with daily living tasks, offer emotional support, and provide meaningful companionship. Your care will ensure clients feel safe, respected, and valued in the comfort of their own environment.
Since this role is fully onsite, there’s no need to travel between clients — just a short, pleasant stroll between apartments and villas across our beautifully landscaped village grounds. It’s a rewarding way to provide personalised care alongside our supportive and collaborative team.
This position is a Casual position offering a variety of shift times including weekends.
With kindness and understanding, you will provide:
Domestic Assistance – Supporting with household chores such as washing, ironing, vacuuming, and dusting.
Meal Preparation – Assisting with planning, preparing, and cooking meals.
Personal Care Support – Helping with showering, dressing, grooming, and other personal hygiene needs.
Social Engagement – Providing companionship and encouraging participation in social activities and community engagement.
Transport Services – Offering transport for appointments, shopping trips, and social outings.

About the Role

About Neurosciences
The Neurosciences Unit at St Vincent’s Hospital includes Stroke, Neurology and Neurosurgery patients across two clinical areas. There are over 300 presentations of stroke per year at SVHM with care provided by a multidisciplinary team in a dedicated 6 bed Stroke Unit. Medical intervention includes hyperacute reperfusion therapy with administration of thrombolysis, and /or Endovascular Thrombectomy (EVT) with neurosurgical, neuro-interventional and vascular surgery co-management as required. Prince of Wales Hospital and Community Health Service is a Level 6 Tertiary Referral Hospital with an inpatient bed base of 370. Each year we care for more than 70,000 patients in our Emergency Department and have around 50,000 admissions to the inpatient units. POWH offers an outpatient service and rural outreach service and provides more than 900,000 occasions of non-admitted patient care each year, including innovative virtual models of care. Garrawarra Centre is a 104 bed Dementia Specific Residential Aged Care Facility located in the tranquil setting of Garrawarra State Conservation Area, Waterfall. The Centre is one of the few purpose-built, dementia specific facilities in New South Wales.
There is ample, free on-site parking but you will need your own reliable transport as Garrawarra is not accessible by any public transport.

The Role
The Enrolled Nurse provides safe, holistic and patient-centred nursing care within the Respiratory & Infectious Disease Unit at Prince of Wales Hospital, working under the supervision of a Registered Nurse and in accordance with NSW Health, SESLHD and professional nursing standards. The role involves assessing and responding to patient needs, contributing to care planning and delivery, maintaining accurate clinical documentation, and communicating effectively with patients, families and the multidisciplinary healthcare team to support continuity of care.
The Enrolled Nurse applies critical thinking within their scope of practice, promotes patient dignity, safety and wellbeing, manages priorities effectively, and maintains current clinical knowledge and skills through ongoing professional development, education and training.

Prince of Wales Hospital and Community Health Service is a Level 6 Tertiary Referral Hospital with an inpatient bed base of 370. Each year we care for more than 70,000 patients in our Emergency Department and have around 50,000 admissions to the inpatient units. What You'll be doing
Provide clinical management and treatment of patients in a SWSLHD facility under your care. This care will be provided within the clinical privileges granted by the CE on the advice of the SWSLHD Medical and Dental Appointments Advisory Committee.As one of the Staff Specialists in this Unit, you will primarily take part in the clinical duties of the TACS unit and cover Trauma and ASU (Acute Surgical Unit) as well as participate in the General Surgery on call after hours roster.Coordination and management of the acute resuscitation and treatment of multiple injured patients, emergency general surgery patients and coordination of acute care between multiple services and disciplines is a major focus of the role.You will also be expected to assist the department in the running of Trauma, General Surgery, and Acute Surgery clinics as well as participate in unit elective and emergency surgical operating lists.
