The Opportunity
This Team Leader role provides leadership and operational oversight across both the Dementia Behaviour Management Advisory Service (DBMAS) and the Hospital to Aged Care Dementia Support Program (HACDSP). The position is responsible for leading a multidisciplinary team that supports people living with dementia, their families, carers, and healthcare providers through behaviour support, dementia care planning, and transitions between hospital and aged care settings. The Team Leader will promote high-quality service delivery, staff development, collaboration, and best-practice dementia care outcomes across both programs.

The Role
Apply your expertise in assessment and biopsychosocial approaches, implement practical, innovative and non-pharmacological strategies to prevent, minimise or respond to changes in behaviour related to dementia.
This role is offered as a Permanent / Temporary (up to June 2028) Full-time opportunity, based out of our Darwin office.
Responsibilities
- Understand and engage with HammondCare Mission in Action, Model of Care, Case Management Framework and practice guidelines, policies and procedures.
- Provision of professional and clinical leadership for team members
- Ensuring timely allocation of incoming referrals to the appropriate consultant in alignment with DSA KPI’s and Activity Work Plan.
- Responsible for office environments including safety, routine maintenance and functioning equipment and ICT.
- Oversees local use and maintenance of fleet vehicles including utilisation, electronic tracking and reporting.
- Responsible to delegated approvals for brokerage and travel, in line with financial delegation and HammondCare Policy.
- Ensure that the wellbeing and safety of team members is considered and proactively managed.
- With support of Regional Manager maintain local key stake holder relationships
- Provide regular reporting to Regional Manager on all staff performance, service quality, operational performances and issues.
- Recruit team members with appropriate skills for the specific roles being recruited for.
- Effectively manage customer complaints or challenging situations as they arise, with early notification to the Regional Manager, and to the Head of National Programs when needed.
- Work with Regional Manager to address incidents and feedback relating to the quality of the DSA Service
- Improving your own knowledge and skills as part of the Advisory Care & Partnerships team.
What do we need from you?
- Tertiary qualifications in Nursing/ Allied Health or health- related degree.
- Demonstrated experience and knowledge in supporting older people and carers living with dementia, with particular emphasis on psycho-social approaches to case management.
- Ability to manage and support a team of skilled professionals across multiple sites and locations.
- Displays strong customer service ethic and attitude
- Ability to work on multiple tasks and projects simultaneously.
- High level communication and interpersonal skills
- Strong planning and organising skills with the ability to prioritise tasks to meet deadlines.
- Strong analytical skills and attention to detail.
- Good customer management skills for delivery of services to HammondCare and external customers
- Strong administrative and computer skills.
- A current Australian driver’s licence and willingness to travel intra and interstate
- Empathy and patience
Desirable:
- Eligibility for registration as a Health Practitioner in Australia
- Evidence of study and or qualifications in the area of dementia
- Knowledge of supporting older people and carers living with dementia

About the opportunity
The Geriatric Evaluation and Management and GEM@Home Unit consists of a state of the art 30 bed inpatient ward with GEM/ TCP/ Rehabilitation that provides specialist in-patient care to the growing communities of the West. The Unit is based at Werribee Mercy Hospital. The unit is supervised by a Head of Unit and is made up of Geriatricians, an Advanced Trainee, BPT registrar, HMO’s and an Intern. There is a strong multidisciplinary team consisting of medical, nursing and allied health staff. In addition to ward responsibilities, you will have the opportunity to be involved in ACAS rounds as well as specialist clinics. Werribee Mercy Hospital is a teaching hospital of the University of Notre Dame.

About the opportunity:
At Mercy Health, each moment, no matter how small, truly matters - and as a Personal Care Assistant, you’ll make a genuine difference when it’s needed the most by:
- Assist residents with daily personal care tasks, including bathing, dressing, grooming, and toileting.
- Help with mobility, including transferring residents from bed to chair, assisting with walking, and using mobility aids safely.
- Support residents with feeding, ensuring proper nutrition and hydration, and assisting with special dietary needs.
- Assist with medication reminders and support residents in taking prescribed medications, under the guidance of a nurse.

- Supporting the achievement of core activities within the Programs expansion project, ensuring that key deliverables in relation to site implementation and referral of eligible participants to new and existing sites are accomplished
- Leading service delivery operations in line with the Project SEARCH Model Fidelity, practice frameworks and legislative, regulatory and licensing requirements
- Working in partnership with the service delivery team to implement and promote best practice in delivering high quality, individualised, outcome focused supports, aligned to each participant’s employment goals
- Developing, implementing and executing an Annual Program Delivery Plan for each designated program site
- Leading the development of a range of internships designed to teach participants competitive and marketable skills that are transferrable to jobs within the community
- Lead service delivery teams in undertaking a comprehensive assessment of each participant to inform Individual Employment Plans, Risk Assessments and individual servicing requirements
- In collaboration with service delivery teams and relevant stakeholders, develop effective employer engagement strategies designed to secure employment opportunities for participants
- Play in active role in the recruitment, on-boarding, induction and ongoing professional development of employees

The Team
The Case Management team forms part of People and Culture within the Department of Health. The team provides specialist advice and guidance across a broad range of employment matters, including employee grievances, complaints, performance management and workplace conflict. The team partners with stakeholders at all levels of the organisation to assess concerns, identify appropriate pathways for resolution and ensure matters are managed consistently and in accordance with relevant policies, procedures and best practice principles.
The Role
As a Case Management Consultant, you will provide high-level advice and consultancy services. The role is responsible for supporting managers and employees across the Department to address complex workplace matters in a fair, practical and solutions-focused way.
You will contribute to the development of policies and procedures, undertake research and analysis on employment-related matters, prepare correspondence, liaise with a broad range of stakeholders, and support the Department in achieving best practice employee relations outcomes.
You will need
- High level knowledge and understanding of contemporary human resource management issues, employment legislation and industrial Awards/Agreements as they apply within the Tasmanian State Service context; together with a sound understanding of the principles of procedural fairness and their practical application in a contemporary HR service.
- Demonstrated skills and experience in providing a high-level HR consultancy service in relation to dealing with complex issues including legislation interpretation, complaint handling and dispute resolution, within the political, social and organisational environment of health and human services in the public sector.

The Team
Royal Hobart Hospital is Tasmania’s largest hospital, and the Adult Diabetes Centre within the RHH is the tertiary referral centre and a member of the National Association of Diabetes Centre in Southern Tasmania. As the major clinical teaching and research centre, RHH works closely with the University of Tasmania and other institutions. All our services work closely together to focus on delivering excellence in healthcare to achieve improved outcomes for our consumers, our workforce, and the Tasmanian community.
The Role
- Demonstrates an interest in diabetes management and ongoing learning.
- Provides support to people living with diabetes, their families, and carers under the supervision of the CNS or CNC.
- Works collaboratively within multidisciplinary teams across THS-S and the RHH Diabetes Centre.
- Contributes to quality improvement activities that enhance clinical practice and service delivery.
- Promotes evidence-based practice through education and professional development.
You will need
- Current knowledge of, and the ability to apply nursing principles, procedures and practices in the delivery of patient/client care with in the RHH Diabetes Centre and in line with legal requirements and the Australian Nursing and Midwifery Board (NMBA) Registered Nurse standards of practice.
- Sound interpersonal and communication skills, and the ability to function effectively in a multidisciplinary team environment.
- Ability to undertake patient/client education in RHH Diabetes Centre, together with a commitment to participate in ongoing professional developing and safety and quality improvements.

Permanent part time, day worker position, working 48 hours per fortnight, commencing from a mutually agreed upon date.

Salary

The role is accredited for RACGP Extended Skills Post and is ideal for GPT 3 and above registrar position.

Position details
The Team
The Sexual Assault Medical Service is a specialised service of both doctors and nurses within the Department of Health’s Women’s & Children’s Service, supporting adult and paediatric clients affected by sexual assault and family violence.
The team works as part of the broader Tasmanian health system to provide safe, accessible, culturally respectful and evidence-based care to vulnerable people. Through multidisciplinary collaboration, clinical leadership, education, quality improvement and strong links with community and justice services, the team supports the Department’s commitment to safeguarding, improving health outcomes, and ensuring coordinated care for individuals and families across the community.
The Role
Provide expert clinical and forensic care for adults affected by sexual assault and family violence, including forensic examinations, evidence collection, medico-legal reporting, court testimony, and support for paediatric assessments. Lead clinical governance, policy development, statewide reporting, and continuous quality improvement to ensure evidence-based practice.
Build workforce capability through education and stakeholder engagement, strengthen partnerships across health, police, child protection and justice sectors, and support compliance, risk management, safeguarding, and service development to deliver high-quality forensic and support services.
You will need
- Highly developed clinical management and leadership skills, and demonstrated advanced clinical expertise in the specific area of forensic and clinical examination in accordance with best practice standards, utilising a high degree of independent decision making, with the ability to perform as an expert witness in relation to evidentiary processes within the criminal justice system.
- Demonstrated highly developed leadership, communication, interpersonal, problem solving and decision making skills with the ability to apply these skills within a sensitive setting and to ensure effective partnerships with internal and external stakeholders.
- Highly developed skills regarding the coordination and integration of contemporary information and research evidence to support decision making, innovative thinking and objective analysis to achieve agreed outcomes within Sexual Assault Support Service setting, including the ability to contribute to the review and development of policies and procedures relevant to the practice setting.

Details of appointment
Permanent part time, day worker position, working 22.8 hours per fortnight, commencing from 19 October 2026.
*notwithstanding hours to be negotiated with the successful applicant.

Salary
$118,297 to $125,543 per annum (Pro rata). Our Employer 12% superannuation contribution is on top of this amount.
- Salary range is in accordance withNurses and Midwives (Tasmania State Service) Agreement 2023.
In addition, the following allowances calculated on the salaried incremental point may be available:
- Post Graduate Allowance of up to 10% based on qualification type

Eligibility
Successful applicants will be required to meet the essential criteria
- Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse
- Current Tasmanian Working with Children Registration

- Holds or is working towards a post-graduate qualification in the area of sexual assault forensic and medical examination.
